Position Overview:  

PCG is looking for an Epic Care Tapestry Analyst who is responsible for the implementation, configuration, and support of the Epic Care Tapestry module within a healthcare organization. This role involves collaborating with various stakeholders, including care managers, social workers, and community health workers, to ensure the Tapestry module aligns with the organization's care coordination and social determinants of health initiatives. The analyst serves as a subject matter expert for care management processes and provides technical support to end-users. 


Key Responsibilities: 

  • Epic Care Tapestry Module Configuration: Configure and maintain the Epic Care Tapestry module based on the organization's care coordination and social determinants of health requirements, including care plans, community resources, referrals, and patient outreach campaigns. 
  • Workflow Analysis and Optimization: Collaborate with care management teams to analyze existing workflows and identify opportunities for streamlining and optimizing care coordination processes, including assessment, goal setting, interventions, and follow-up. Implement changes to improve efficiency, patient outcomes, and the integration of social determinants of health. 
  • Data Integration and Analytics: Coordinate with internal and external data sources to ensure accurate and timely integration of patient data, including social determinants of health, into the Tapestry module. Utilize analytics tools to identify care gaps, track outcomes, and support population health management initiatives. 
  • Referral Management and Coordination: Implement and maintain referral management workflows within the Tapestry module to facilitate efficient and effective coordination of care across multiple providers and community resources. Collaborate with stakeholders to ensure seamless referral processes. 
  • Training and Support: Develop and deliver training programs to educate care managers, social workers, community health workers, and other end-users on the use of the Epic Care Tapestry module. Provide ongoing support, troubleshoot issues, and resolve user inquiries in a timely manner. 
  • Regulatory Compliance: Stay updated with relevant regulatory guidelines and requirements related to care coordination, social determinants of health, and value-based care initiatives. Ensure the Tapestry module meets these requirements and assist in the documentation and reporting of quality measures. 
  • Documentation and Reporting: Maintain comprehensive documentation of workflows, system configurations, and changes made to the Tapestry module. Generate reports and provide data analysis to support decision-making processes and performance improvement initiatives. 
  • Continuous Improvement: Stay abreast of industry trends and best practices related to care management, social determinants of health, and EHR systems. Recommend enhancements and improvements to the Tapestry module to enhance care coordination, patient engagement, and health outcomes. 
Requirements

 

  • Bachelor's degree in healthcare informatics, nursing, social work, public health, or a related field. Relevant certifications in Epic Care Tapestry are highly desirable. 
  • Prior experience working with the Epic EHR system, specifically in the configuration and optimization of the Tapestry module. 
  • Strong understanding of care coordination processes and social determinants of health concepts, including the integration of community resources and referrals. 
  • Knowledge of healthcare data sources, including social determinants of health data, and experience integrating data into care management systems. 
  • Proficiency in analytics tools and techniques to support care management and outcomes tracking. 
  • Excellent analytical and problem-solving skills with the ability to interpret complex data and identify actionable insights. 
  • Strong communication and interpersonal skills to collaborate with cross-functional teams, stakeholders, and end-users. 
  • Ability to prioritize tasks, meet deadlines, and work in a fast-paced environment. 
  • Attention to detail and a commitment to maintaining data integrity and patient privacy.
