Chamber Cardio seeking a Cardiology Care Coordinator to join the care team. The Cardiology Care Coordinator provides care coordination services to meet medical, behavioral health and social needs of members while promoting and maintaining the highest quality. Join this growing and dedicated team working to improve cardiovascular health. 

Responsibilities: 

  • Conducts regular assessments of members’ medical, behavioral and social needs. 

  • Builds constructive working relationships with physicians, social service providers, health plans and other office staff by involving them in the Care Coordination process 

  • Schedules patients and notifies them of appointment information, including, date, time, location, etc. 

  • Uses Chamber Platform and other applications to review, monitor and update care plans as indicated. Assists practices with documenting in Chamber Platform and any associated platform. 

  • Provides information to members and providers regarding benefits, service providers and access protocols and educates members on appropriate use of medical services. 

  • Provides referrals for community resources and social services as necessary. 

  • Documents member information and contacts made regarding member’s care and services provided. 

  • Actively monitors member’s ability and willingness to engage in treatment regimes, and problem solves alternatives or solutions to their needs. with staff, members and providers, 

  • Conducts follow up calls post hospitalization and emergency room visit according to policy. 

  • Educates members on preventive health guidelines and programs. 

 

Qualifications: 

Required: 

  • High School diploma or equivalent required 

  • A minimum of 2 years of relevant healthcare experience in a clinical setting 

Preferred: 

  • Knowledge of medical terminology 

  • Detail-oriented with the ability to multi-task. Must be open to cross-functionally training in referrals and back office duties 

  • Able to exercise proper phone etiquette with the ability to navigate proficiently through computer software systems 

  • Team-oriented with the ability to work extremely well with patients, colleagues, physicians and other personnel in a professional and courteous manner 

  • Exceptional organizational skills with the ability to effectively prioritize and timely complete tasks 

  • Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, database 

  • Ability and willingness to travel locally within the market 

  • Spoken and written fluency in English; Bilingual a plus 
     

Additional Information 

  • Workstyle: This is a hybrid position where employees mostly work on-site with physician offices but may have days working from home 

  • Work Location: must reside Washington D.C. or Maryland 

  • Travel: Requires Regular Travel to Physician offices 

  • Typical Work Days/Hours: Monday – Friday, 8:00 am – 5:00 pm EST
