What You’ll Do 

We’re excited to be adding a Transitional Services Manager to our growing team! This is an engaging and rewarding opportunity that plays an integral role within the organization. We are looking for someone who is passionate about working with our participants and contributing to the fight against homelessness. The Transitional Services Manager will report directly to the Director of Residential Services and be part of the Hamilton Transitional team. This is a role requires strong leadership, organizational skills, and the ability to provide support to a diverse group of participants. The Transitional Services Manager will ensure that the Transitional Housing Program runs smoothly while also overseeing the operational functions of the program including oversight of the maintenance support, case management services for families, guiding them through intake, assessments, and resource connection to help them transition from homelessness to stability. The ideal candidate will have experience working with children, families, and be a liaison to city funders and stake holders. 

What Your Day Looks Like 

• Develop, implement, and manage community programs that support families and individuals impacted by trauma
• Apply trauma-informed principles to all aspects of program development, delivery, and evaluation
• Design and facilitate workshops addressing parenting, family resilience, mental wellness, and other community-identified needs.
• Monitor and evaluate program outcomes; use data to improve services and report on program impact
• Communicate with families about program opportunities and community resources
• Lead training sessions for staff and partners on trauma-informed care, best practices in community engagement, and program-specific topics
• Provide support to the Case Management team in providing personalized services to families, including intake, assessments, and individualized plans
• Support in collaboration with the Family Service Manager and Case Managers to connect families with community resources like housing, employment, mental health, and education
• Collaborate with the Children’s Services team to plan and implement events and activities, including field trips and holiday events
• Oversee the preparation of progress reports, HSH annual city monitoring, support internal financial audits, ensure accurate of program documents, including Program Agreement, timely documentation of participant records through internal platforms and citywide One System and Carbon monthly, quarterly and yearly report in collaboration with the Director of Residential Services as needed
• Ensure program activities for families comply with organizational policies and confidentiality requirements
• Build relationships with community partners to advocate for families’ access to necessary services
• Collaborate with Shelter and THP management team in support of workshops on life skills, crisis intervention, and empowerment to help families achieve long-term success
• Collaborate with Shelter and THP management on oversight of Grievance and compliance process. Ensuring Standards of Care are being met and ensuring the safety component of program
• Lead regular team meetings and case reviews to keep staff informed, supported, and aligned with program goals
• Model professional boundaries, confidentiality, and ethical standards
• Respond to emergencies, coordinating with authorities as needed
• Participate in staff development, training, and team meetings to foster a positive work environment
• Support evening and weekend program events to ensure program success
• On-call support to program when needed
• Contribute to developing strategies to improve program effectiveness and participant engagement
• Perform other duties as requested

What You Bring  

• Bachelor’s degree in social work, Psychology, or related field preferred.
• Experience working with children and families, preferably in a shelter or homeless services setting
• Knowledge of community resources for low-income families, including housing, employment, and social services
• Proven ability to lead and motivate a team, balancing multiple priorities effectively
• Demonstrated understanding of trauma-informed care and sensitivity to the needs of individuals and families experiencing homelessness
• Bilingual abilities, especially in Spanish, preferred.
• Current CPR and First Aid Certification (or willingness to obtain).
• Proficient in Microsoft Office Suite, Salesforce, and other relevant software.
• Ability to travel locally and long distance to meet with participants, vendors, and partners.

Salary Range: $80,000 - $100,000 annual salary 

Position Type: Exempt

Schedule: Monday -Friday + some weekends
