Principal Objective/Role Overview:

Graham Windham seeks dynamic child/youth health advocates to join our Children’s Health Home Care Management service.  The Graham approach to Care Management is family focused and supports a strength-based system of care. Our Health Home program is structured in Care Management Teams composed of a Supervisor and five (5) Health Home Care Managers.  Teams will be in the Bronx and Brooklyn.

Health Home Supervisors set an example through their own delivery of Care Coordination and in developing the talent of their Care Management team. Supervisors will ensure that there is timely access to care, continuity of care and comprehensive transitional care from service to service and across and within systems (education, foster care, juvenile justice, child to adult).  Supervisors will ensure consistent use of standardized tools to monitor functioning and need for clinical interventions.  Supervisors will also ensure consistent communication around the Care Plan with caregivers, providers, case planners and other stakeholders in the youth/family’s life.

 

Job Responsibilities:

  • Direct Health Home Care Managers for an identified cohort of between 5 to 10 children.
  • Responsible for coaching and supervision of a team of five (5) Care Managers in the performance of the major functions of a Health Home: including,
    • Comprehensive Care Management
    • Care Coordination and Health Monitoring Comprehensive Transitional Care
    • Individual and Family Support
    • Referrals to Community and Social Support Services
    • Health Information Technology.
  • Comprehensive Care Management:
    • Monitor enrollment into Health Home and ensure that necessary documents are obtained on a timely basis by Care Managers.
    • Reviews Care Plans for quality control to ensure that all health needs are being met based on assessments and provider recommendations.
    • Ensures that CANS-NY instruments are completed yearly for all enrolled children/youth they are monitoring.
    • Maintains CANS-NY certification on an annual basis.
    • Monitors Care Managers’ completion of monthly care management billing to the Health Home.
  • Care Coordination and Health Monitoring:
    • Tracks that there is consistent coordination provided by Care Managers between Graham Windham case planning, caregivers, and community providers to deliver health care services including: primary and specialized care, dental care, optical, behavioral health and pharmacological.
    • Ensures that enrollees medical/behavioral health treatment recommendations are in place for enrolled youth.
    • Ensures that Care Managers are soliciting input from caregivers, case planners and providers as to the success of the Care Plan.
    • Supports continuity of care.
  • Comprehensive Transitional Care:
    • Ensures that Care Managers notify multidisciplinary team members of admission and/or discharge from Emergency Room, inpatient hospitalization and/or residential care settings.
    • Ensures that Care Managers complete Mental Health Coordination Unit admission, follow-up and discharge forms for psychiatric hospitalizations based on prescribed time frames.
    • Monitors transition with services following final discharge.
  • Individual and Family Support:
    • Ensures that Care Managers continuously work to engage and communicate with the enrollee, case planners and families. 
  • Referrals to Community and Social Support Services:
    • Creates and maintains strong relationships with neighborhood-based providers to whom children and youth can be referred for community health services. 
    • Ensures that Care Manager performs follow-up monitoring to ensure that services are delivered promptly and consistently.
    • Monitors Care Manager’s follow-up with regards to provider treatment recommendations (i.e. referrals, evaluations) and ensures that follow-up is scheduled accordingly. 
  • Health Information Technology:
    • Reviews/monitors documentation of Care Management activities and ensures that all activities are properly documented in a timely fashion. 
    • Reviews Care Managers’ documentation in Connections and ensures accuracy and timeliness. 
    • Ongoing reviews of medical records to ensure they meet standards of care and regulatory guidelines.
  • Program Administration
    • Responsible for staff schedule (i.e. shift schedules, approval of vacations/time off requests) and approval of timesheets on a biweekly basis. 
    • Responsible for bi-annual performance evaluation of staff and consistent feedback of performance in structured supervision. 
    • Responsible for referring and scheduling staff for skills training based on evaluation of performance and staff needs.

 

Qualifications

Position Qualifications:

  • BA/BS with 2 years of relevant experience, OR a License as a Registered Nurse with 2 years of relevant experience OR Masters with 1 year of relevant experience. 
  • Two (2) years of supervisory experience preferred.
  • Familiarity with NYS Connections system a plus.
  • Bilingual Spanish/English preferred.
  • Willingness and ability to travel throughout the five boroughs of New York, and Westchester County.
  • Ability to work flexible hours according to the program’s need.
  • Computer Literacy including Microsoft Office.
  • Detailed oriented, with excellent verbal and written communication skills.
  • Excellent time management and organizational skills.

  • Demonstrated ability to collaborate within a team and to build and maintain strong relationships with internal staff and external care providers.

Salary & Compensation:

  • Base Salary or Hourly Rate: $65,000.00/year
  • FLSA Status: Exempt
