Description

POSITION SUMMARY: Responsible for serving as liaison for and among the Mental Health Court for both in and out of county residents. Assisting social work team with discharge planning for patients to support successful transition back into the community and reduce the risk of readmission.

PRIMARY DUTIES:

  • Serves as liaison between the Psychiatrists, Mental Health Review Officers, Public Defenders, County delegates and patients.
  • Follows-up on commitment status changes to ensure all patients remain on a valid commitment.
  • Plans and facilitates all meetings associated with Mental Health Court, including treatment team meetings, stakeholder meetings, commitment hearings and additional, as requested.
  • Complete patient safety crisis plans.
  • Identify healthcare disparities and reduce barriers.
  • Establish aftercare appointments.
  • Performs data entry by updating and maintaining all excel spreadsheets, in-house logs and hearing schedules to ensure all patients remain on a valid commitment.
  • Assures all paperwork and documents for all Involuntary Commitment types are assimilated into the Mental Health Court process timely.
  • Uploads court commitments into HCS system.
  • Coordinates the completion of all petitions, orders, continuances and ACT 77 forms timely.
  • Assist with all aspects of discharge documentation such as patient satisfaction surveys, healthcare disparity discharge form, Medicare right to appeal, etc.
  • Works with the Mental health Court systems for the counties to file documents timely.
Qualifications

Education:  Bachelor Degree in social work or related clinical field.

Experience:  A minimum of one (2) year experience in the mental health field required; thorough understanding of the Mental Health Procedures Act and ability to identify valid commitment types. Awareness of services available to patients and their families.

Certifications/Licenses: CPR / CPI per position requirements; Valid Driver’s License.
