Program Description:

Graham’s Care Coordination team plays a vital role in supporting the health and well-being of children and youth in foster care. Our work ensures that every child—no matter their background or circumstance—receives coordinated, high-quality medical, behavioral, and mental health care. We work closely with caregivers, service providers, and city and state partners to break down barriers and build up systems of support, all grounded in trauma-informed and family-centered care. A subset of this work includes our Managed Care Liaison (MCL) function under our 29-I license, which ensures timely, effective coordination between Medicaid Managed Care Plans, providers, and caregivers—facilitating access to services, resolving coverage issues, and supporting continuity of care for children and youth.

 

Principal Objective/Role Overview:

This is a vital position within Graham’s Care Coordination (29i) Program. The Managed Care Liaison Supervisor is responsible for strengthening the operational and clinical effectiveness of the program by overseeing Medicaid Managed Care alignment for children in foster care. This role ensures that each eligible child has appropriate and uninterrupted insurance coverage and that Graham is positioned to deliver and bill for critical medical, behavioral, and mental health services.

Reporting to the Associate Vice President of Behavioral Health Services, the Supervisor oversees a team of Managed Care Liaisons (MCLs) who manage the day-to-day functions of Medicaid coverage, enrollment, issue resolution, and provider coordination. This includes monitoring for changes in placement, eligibility, and service access. The Supervisor collaborates closely with internal teams (Care Coordination, EFFC, Fiscal, and IT) and external stakeholders (Managed Care Organizations, MMCP Foster Care Liaisons, and State agencies) to build sustainable workflows that uphold service access and ensure reimbursement.

This position is ideal for someone who thrives in a systems-oriented environment, with a strong ability to translate complex policy into actionable practice. The successful candidate will bring expertise in Medicaid Managed Care and foster care systems, along with excellent organizational, interpersonal, and analytical skills. A passion for advocacy, operational excellence, and continuous improvement is essential to succeed in this role.

 

Job Responsibilities:

  • Develops and maintains strategic relationships with all Medicaid Managed Care Organizations (MMCOs) and FCLs contracted with Graham.
  • Oversees the maintenance of continuous health insurance eligibility—Medicaid, public, or private—for children throughout their foster care placement.
  • Leads the design and implementation of systems, in partnership with IT and program staff, to regularly track and verify health insurance eligibility.
  • Coordinates discharge planning efforts to ensure continuity of medical coverage for children transitioning out of care.
  • Ensures ongoing communication with program and billing teams regarding changes in Medicaid status and eligibility.
  • Engages parents and foster parents to provide updates and education around Medicaid coverage and third-party insurance.
  • Manages timely and accurate notifications to MMCOs regarding placement changes, billing concerns, consent/confidentiality issues, and court orders.
  • Supervises a team of Managed Care Liaisons, providing oversight, support, and direction to ensure operational effectiveness and adherence to coverage protocols.
  • Demonstrates working knowledge of MCL responsibilities and can step into those functions as needed to ensure continuity and coverage during gaps or complex cases.
  • Carries a partial MCL caseload to maintain firsthand knowledge of systems and workflows, and to directly support coverage, eligibility, and reimbursement activities.

  • Bachelor’s degree in healthcare administration, public health, social work, or related field
  • Supervisory experience, ideally in a healthcare, behavioral health, or child welfare setting
  • Excellent written and oral communication skills
  • Proven ability to manage multiple priorities to completion
  • Demonstrated success in building strong internal and external partnerships
  • Ability to interpret and operationalize policy into clear, effective workflows
  • Systems-oriented problem-solver who identifies root causes and drives sustainable solutions
  • Strong working knowledge of Medicaid, Managed Care Organization operations, and 29-I Health Facility regulations
  • Deep understanding of the foster care system and the complex medical and behavioral health needs of youth in care

 

Preferred Skills

  • Familiarity with state data systems such as NYS ePACES, NY Medicaid, and Connections
  • Proficiency with Microsoft Excel and Power BI for data analysis
  • Experience working in a 29-I Health Facility or managed care coordination role

 

 

Salary & Compensation:

  • Base Salary or Hourly Rate: $70,000.00/year
  • FLSA Status: Exempt
