Description

Join Our Team as a Home Health Physical Therapist (PT).

Qualifications for the position include:

 

  • Graduate of a physical therapy school approved by the Council of Medical Education of the AMA or its equivalent
  • Licensed or registered to practice as a PT in the state of practice
  • Preferably at least two (2) years of experience as a PT,  supervising a Physical Therapist Assistant in Home Health
  • Extensive and diverse training in areas such as orthopedic, neurology, cardio-pulmonary, etc.
  • Ability to read and follow written instructions and document care given
  • Self-directing and able to work with little direct supervision
  • Possesses and maintains current CPR certification
  • Flexible and cooperative in fulfilling role obligations
  • Demonstrates tact, patience, and good personal hygiene
  • Licensed driver with an insured automobile in accordance with Lorian Health requirements
  • Ability to verbally communicate in the language of the client and document all client related activity practices.

We are a Pay Per Visit compensation model. Pay Per Visit rates range from $70/visit - $140/visit commensurate with experience and depending on type of visit.  The compensation reflected on this posting is an estimate of yearly compensation.

As a Home Health Physical Therapist (PT),you will play a vital role in providing care to our patients in various communities.  You will work closely with physicians, to deliver prescribed physical therapy services to patients in the comfort of their own homes.

In this position, you'll travel throughout the day to patients' homes, delivering personalized physical therapy services. You'll use our provided electronic device (smartphone) for documentation, ensuring accurate and up-to-date records.

 

Your responsibilities will include:

 

  • Providing treatment as ordered by the attending physician, dentist, or podiatrist
  • Assisting in developing and updating the plan of treatment or Plan of Care (POC) in an electronic medical record system
  • Supervising 1 to 2 Physical Therapy Assistants, ensuring productivity standards are met
  • Evaluating patients' level of function and reporting findings to the attending physician and case manager
  • Advising, consulting, and instructing family members and Lorian personnel on patients' therapy program
  • Participating in in-service education programs to enhance your professional development
