Salary Range:    $25.00 - $25.00 Hourly
Description
Work in conjunction with Shared Living Director and Shared Living caregiver to provide support, evaluation and treatment to individuals with developmental disabilities living in the caregivers home. This position follows a hybrid schedule consisting of three 4-hour weekday shifts from 5:30 AM to 9:30 AM, and one 8-hour shift on Saturdays from 8:00 AM to 4:00 PM.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

  • Write progress notes each time staff takes individual out into the community for support hours.
  • Support needs may include personal care responsibilities such as: dietary needs, exercise, meal preparation, bathing, shaving, dressing, toileting etc. Willing to become certified on G-tube administration and cleaning.
  • Physical support for individuals may include: physical or mechanical transfers from wheelchairs to van, chair or shower etc. Assistance with individuals for low vision trailing within home and community.
  • Maintain communication with Shared Living Residential Director as appropriate. There will generally be at least monthly communication 
  •  Adhere to all established lines of communication, dress in a professional manner and represent a positive and proactive attitude with individual’s family members and caregiver.
  • Responsible for writing documentation utilizing professional and appropriate documentation skills.
  • Attend all required meetings.
  • Adhere to agency safety standards.
  • Perform additional duties, or substitute for others as required.
  • Adhere to all RTR policies and procedures.
  • Develop and provide all services in accordance with RTR’s philosophies including staff and Board beliefs.
Qualifications

  • High School Diploma or equivalent.
  • Valid Driver’s License.
  • Basic knowledge of Microsoft Word and the internet.
  • Successful completion of all state and agency mandated trainings.
  • Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies.
  • Must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.

 

LANGUAGE SKILLS

Must be fluent in English and have the ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to communicate effectively with individuals under supervision, employees of the organization, families, and members of the community.
