Developmental Pathways (DP) has an opportunity for a Case Manager– Active in our Case ManagementDepartment. The Case Manager – Active will manage a caseload of individuals receiving Waiver and State General Fund services and maintain documentation needed quarterly and annually. The Case Manager – Active will complete assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools. The Case Manager – Active will complete the required documentation related to monitoring and monitoring activities to advocate for individual service needs and ensure the effectiveness of services delivered (as authorized by the service plan). The Case Manager - Active will facilitate interdisciplinary teams (IDT) meetings to coordinate services to meet individual needs; ensure appropriate interaction and communication; and as appropriate to modify services. Lastly, will provide case management services and support to temporarily assigned individuals; and support with department and team coverage as needed.
This is a 40-hour per week, hybrid position, with the benefit of working remotely and in-person coming into our headquarters office in Aurora, CO and/or off-site locations

Five (5) years of experience in the field of Long-Term Services and Supports; or
(LTSS), which includes Developmental Disabilities; or
Some combination of education and relevant experience appropriate to the requirements of the position
Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years
Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

Additional Responsibilities
Schedule, facilitate, document, and complete follow-up for all required meetings; disseminate updates and obstacles to the necessary team members
Make collateral contacts with such community resources such as law enforcement and medical personnel to obtain additional information, coordinate services, or secure their assistance
Ensure the accuracy and timely entry and distribution of required documentation to all necessary team members, including, but not limited to providers, individuals, and family members
