Description

SUMMARY:

The Care Management Coordinator implements and oversees the care management program referrals and performance. This person ensures that program is meeting key performance indicators and organizes quality improvement and oversight initiatives to address performance issues. This person manages the incoming referrals and coordinates referral linkage to internal programs. This person provides educational presentations to sites and engages providers to identify and engage eligible patients. 

 

REQUIREMENTS:

  • Manages referral queue and meets referral assignment expectations
  • conducts monthly performance reviews of assigned panel
  • works collaboratively with care management program leadership 
  • Assists, develops and participates in quality assurance activities and promotes high quality, comprehensive, effective patient care. 
  • Facilitates ongoing chart reviews to ensure quality indicators are met.
  • Participates and attends community meetings/conferences, and conducts trainings.
  • Coordinates mandatory staff meetings.
  • Arranges task and clinical supervision for program staff.
  • Monitors service provision requirements as they relate to health home billing
  • Participates in and helps in the coordination of staff development programs and in-service trainings.
  • Manages all care management referrals, ensuring timely assignment and outreach.
  • Assists in the preparation and updating of policy and procedure manuals and health education materials.
  • Demonstrated organizational, interpersonal, oral and written communication skills and the ability to handle multiple assignments at any time
  • Ability to generate, interpret and analyze data from multiple sources

QUALIFICATIONS

  • Bachelor’s Degree in Social Work, Human Services, Psychology, Nursing or related field preferred
  • Previous supervisory experience (one year) preferred
  • Experience in a health care setting
  • Experience in program coordination and oversight
  • Basic computer and internet navigational skills
  • Computer literacy with Windows-based operating systems and MS Office applications (Word, Excel, Outlook, PowerPoint)
  • Familiarity with electronic health records (EHR)
  • Bilingual Spanish/English
