Job Description

PURPOSE OF POSITION:

Director of Nursing (DON) for Service Delivery – Clinical Services will be responsible for providing oversight of a team Assistant Director of Nursing as well as nursing staff who provide oversight to a clinical team in the coordination and rendering of care delivery in the Certified Home Health Agency (CHHA). The DON for Service Delivery – Clinical Services will be responsible for the delivery of competent, effective and safe clinical services, including but not limited to skilled nursing, as well as other interdisciplinary services. In addition, the DON for Service Delivery – Clinical Services will engage in efficient advisement, coaching and mentoring strategies to the members of the care team in order to meet clinical and professional goals. The DON for Service Delivery – Clinical Services will stay abreast of industry trends and regulatory guidelines that govern the home health care team and clinical practice.

The Director of Nursing for Service Delivery and Clinical Services is part of the leadership team that will focus on ensuring that the home care department, maintains a collaborative relationship with the acute and post-acute care facilities within the health system.
