Job Description
PURPOSE OF THE POSITION:
Under supervision of the Associate Director of Social Work or his/her designee, is responsible for the management and supervision of selected Social Work Services of inpatient and outpatient medicine and psychiatry service areas. Acts a clinical supervisor and provides advisory services related to community issues and resources.
The Social Worker Level III assumes the responsibility as a patient and family experience ambassador, on behalf of Metropolitan Hospital and Health and Hospital Corporation, while on duty. In accords with our first guiding principle, \"Keep Patients First,\" he/or she is expected to handle any patient or family request within their scope. In accord with our second guiding principle of \"Work Together\", if the director cannot handle the matter directly, he/or she is obligated and expected to transfer or escalate the matter to the appropriate staff member. Transfer of responsibility occurs AFTER ONLY speaking directly with the staff member who will assume responsibility for the matter.
Tasks And Responsiblities
Provides clinical and administrative supervision to 2-4 Social Work staff as assigned. Ensures adequate coverage and continuity of patient care at all times.
Provides direct service to all patients on his/her caseload and adheres to Department’s documentation policies and procedures.
Collaborates with all members of the interdisciplinary team to develop and implement treatment plans and post hospital care plans.
Coordinates staff activities to meet the needs of the unit and other programs of the Departments of Psychiatry, Medicine and Social Work such as Koskinas.
Participates in the formulation of standards, policies, and procedures for orientation, supervision, and in-service training of Social Work staff.
Provides orientation to new staff and updates incumbent staff of changes in policy, procedures, or standards.
Provides information about and acts as linkage to available community resources.
Ensures that staff documents all interventions and continuing care plans in the patient’s medical record in accordance with departmental and hospital policies.
Collaborates in completing performance evaluations for staff.
Gathers information as needed to complete monthly statistical reports for both the Hospital and other agencies as required.
Participates in the Department’s cross training program
Conducts and/or attends individual, unit, departmental, intra/inter hospital, and community meetings as mandated by the administration and or supervisor.
Supervises social work interns as required by the Social Work Department.
Reviews Medical Records for assigned areas as indicated by the Social Work Department
Performs other duties assigned by supervisor as needed.
Knowledge And Skills
Able to supervise Social Workers, Caseworkers and social work students
Able to engage patient/family members/significant others to obtain pertinent psychosocial information and develop an appropriate social work/discharge plan.
Able to utilize clinical skills to assess patient’s ability to function in the community.
Able to communicate proficiently (verbally and in writing) in the English language.
Able to communicate verbally and in writing with patient/family member/significant other, supervisor and the interdisciplinary team.
Able to utilize interpretation tools or services available to communicate with non-English speaking patients and families.
Able to document effectively all interventions and continuing of care plans in the patient’s medical record in a clear, concise and legible manner.
Able to type and utilize electronic medical records
Able to prioritize and accomplish multiple tasks.
Able to understand the impact of disease process on psychosocial functioning.
Knowledge of social work techniques in working with individuals, groups and families.
