Work closely with Clients, caregivers and departments across the care center, to ensure the goal of providing a seamless and satisfactory Client experience is achieved, while ensuring that our Clients are receiving the highest quality of care.
Under the supervision of and in consultation with the Director of Operations and Director of Nursing- responsible for coordinating the schedules of all designated clients and caregivers, monitoring schedules, troubleshooting problems and responding to all Client or caregiver needs as they arise.
What You Will Do:
Schedule and coordinate 100% of assigned Client case load in an efficient, consistent and timely manner, while ensuring outstanding customer satisfaction.
Communicate openly with other team members in the department regarding scheduling, attendance, Client changes in condition, and escalating larger-scale issues as determined for prompt resolution and follow up.
Verify caregivers' attendance daily ensuring they are attending their scheduled cases and be able to prioritize staffing cases for no-shows, call-outs, or any missed visits based on the Client's needs of care
Handle and respond promptly to all incoming Client and caregiver calls and email
Communicate with Clients and their families regarding appointment scheduling, plan of care updates, any and all changes in schedule
Maintain and document communication with all Clients in the specified home health software
Coordinate the caregivers' schedules and maintain a schedule of all services rendered per month
Verify billed services against requested services, working in tandem with the Finance Department
Maintain Client confidentiality and privacy with dignity and respect
Ensure compliance with DOH regulations and internal policies and procedures
Ad hoc projects as needed
Who You Will Be:
Bachelor's degree in health sciences, social work or nursing preferred.
2+ years' of experience in home care, healthcare or human/social services industry
Ability to manage a caseload in a fast-paced environment
Ability to multitask
History of achieving the high customer service satisfaction rates
Excellent communication abilities (written and oral)
