Work Schedule

Full Time, Monday through Friday, 8am to 4:30pm plus 1 weekend of call per month and weekday call rotation as needed

Qualifications

Associate or bachelor's degree in nursing required
Current MI RN license is required.
A minimum of one year current, continuous experience in an acute care setting is preferred.
Must be able to read, write, speak and comprehend English.
A current Michigan driver’s license and availability of a car for business use on a daily basis is required, along with proof of auto insurance.
The RN must be able to participate in the lifting and manipulation of patient weights in excess of 100 pounds.

Responsibilities

Calls patients before visits and/or arranges visit time with the patient/care giver.
Completes OASIS information and performs the comprehensive assessment at SOC, resumption, transfer, death in the home, and at discharge.
Assures laptop documentation is appropriate for progress notes and all other documentation.
Initiates and updates the plan of care as appropriate, advising the physician of changes in the patient’s condition. Implements physician orders.
Coordinates services and staffing to facilitate holistic patient care. Problem solves with associates for successful patient outcomes.
Initiates referrals and case conferences with other disciplines as needed. Attends multi-disciplinary conferences and shares information with other care team members.
