Key responsibilities:

Job description

Work Schedule

Full Time, Monday through Friday, 8am to 4:30pm plus 1 weekend of call per month and weekday call rotation as needed

Qualifications 

  • Associate or bachelor's degree in nursing required
  • Current MI RN license is required. 
  • A minimum of one year current, continuous experience in an acute care setting is preferred. 
  • Must be able to read, write, speak and comprehend English. 
  • A current Michigan driver’s license and availability of a car for business use on a daily basis is required, along with proof of auto insurance. 
  • The RN must be able to participate in the lifting and manipulation of patient weights in excess of 100 pounds.

Responsibilities 

  • Calls patients before visits and/or arranges visit time with the patient/care giver.  
  • Completes OASIS information and performs the comprehensive assessment at SOC, resumption, transfer, death in the home, and at discharge.
  • Assures laptop documentation is appropriate for progress notes and all other documentation.
  • Initiates and updates the plan of care as appropriate, advising the physician of changes in the patient’s condition. Implements physician orders.
  • Coordinates services and staffing to facilitate holistic patient care. Problem solves with associates for successful patient outcomes.
  • Initiates referrals and case conferences with other disciplines as needed. Attends multi-disciplinary conferences and shares information with other care team members.
