Linea is transforming post-hospital discharge management for heart failure patients to reduce readmissions and improve patient outcomes. With heart failure being a major driver of preventable hospitalizations—over 40% of patients are readmitted within 90 days—Linea partners with ACOs, health systems, and payers to improve care coordination, reduce costs, and enhance patient engagement.
We are seeking a Head of Sales to build and scale our MSSP ACO and risk-bearing entity (RBE) sales strategy. This role will be responsible for driving top-of-funnel engagement, closing partnerships, and expanding Linea’s footprint across value-based care organizations. The ideal candidate has deep experience selling into ACOs, Medicare Advantage plans, and risk-bearing provider groups, understands HEDIS quality measures and VBC models, and is comfortable navigating payer-provider dynamics.
This is a high-impact, high-autonomy role, working closely with the CEO and leadership team to shape Linea’s go-to-market strategy and refine our sales approach.
