**Your Role**

- **80% Telehealth, 20% Home Visits** *(flexible schedule based on patient needs)*

- Diagnose, treat, and manage **chronic conditions in seniors**

- Provide **preventative care, wellness checks, and medication management**

- Educate patients and families on **healthy aging and chronic disease management**

- Document encounters in **Electronic Health Records (EHR)**

- Collaborate with **physicians, specialists, and caregivers** to enhance patient outcomes

**Who We're Looking For**

✔ **Licensed Nurse Practitioner (NP) in California**

✔ **FNP or AGNP certification** (new grads encouraged to apply!)

✔ **Passion for senior care, home health, and telemedicine**

✔ **Comfortable with technology** (EHR, telehealth platforms)

✔ **Great communicator & patient-centered approach**

✔ **Valid driver’s license & ability to travel within LA for home visits**
