  • Salary: $73,427 - $104,752 + Super + Salary Packaging

About the Role:

  • Permanent, full- time providing mental health case management and comprehensive social work services, including psychosocial assessments and risk evaluation, to patients, families, and carers in our community mental health service.

  • Develop and implement individualised treatment and care plans in collaboration with the multidisciplinary team.

  • Promotion of and commitment to the implementation of quality improvement activities related to the care and treatment of mental health consumers and their carers.

  • Lead safe discharge planning, coordinating referrals to community services and accommodation placements.

  • Liaise with and develop partnerships alongside relevant government and NGO agencies; provide consultation and support; and coordinate with internal and external services.

  • Maintain accurate clinical documentation; actively participate in multidisciplinary team activities; and perform administrative tasks to support these goals.


About You:

  • Degree in Social Work and eligible for membership of the Australian Association of Social Workers (AASW)

  • Experience in community mental health, with confidence in acute and triage settings, risk assessment, and crisis intervention.

  • Committed to holistic, person-centred practice that includes families and carers as part of the care team.

  • A natural collaborator who builds strong, purposeful partnerships across services to deliver seamless, compassionate wraparound support for complex caseloads

  • You’re highly organised, with excellent communication skills and confidence using electronic medical records to manage complex caseloads.
