Posting Salary Range: $85,933 - $100,784

What We’re Looking For

Trillium Health Resources has a career opening for a TCL Manager to join our Transition to Community Living Team. The TCL Manager ensures staff implement industry standards, evidence-based and best practices to support members. The TCL Manager provides oversight to ensure members receive whole person care focused on all members’ needs including physical health, behavioral health, and social determinants of health that helps to establish positive outcomes for members and transform their lives for the better. On a typical day, you might:

  • Ensure staff utilize workflows as required to complete health risk assessment and care plans for members as care needs change.
  • Provide clinical review and oversight of functions within TCL to include screening, diversion, discharge/transition planning, supported housing, and care coordination/care management.
  • Ensure staff are engaging members in care coordination/care management process with reflective listening and motivational interviewing.
  • Provide clinical review and oversight of post-transition activities for members who have transitioned into supported housing.
  • Collaborate with landlords and advocate for individuals to maintain supported housing.

Qualifications: 

Education:

A Master’s Degree is required.

 

Preferred Education:

Human Services degree.

 

Experience:

Three (3) years of experience working with behavioral/mental health, substance use, and/or intellectual/developmental disability populations.

 

Preferred Experience:

  • Work experience focused on working with adults in behavioral/mental health, substance use, and/or intellectual/developmental disability populations
  • Experience supervising a team (2 or more employees)

 

License/Certification:

Must have a valid driver’s license

Fully licensed in North Carolina as a LCSW, LCMHC, LCAS, LMFT, or LPA.

 

Location:

  • Remote within Trillium’s catchment area which includes North Carolina counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, Wilson, Carteret, Craven, Duplin, Jones, Lenior, Onslow, Pender, Sampson, Wayne, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, Scotland, Anson, Guilford, Montgomery, Randolph, and Richmond.
