As we expand our clinical operations to support a growing patient and client base, we are seeking full-time registered nurses (RNs) to join our remote care team. This role plays a key part in our remote physiological monitoring (RPM) program by providing ongoing clinical oversight, developing patient care plans, and offering support and education to help patients improve their health outcomes.

Key Responsibilities

  • Monitor and review patient vitals and care plans using our proprietary technology platform;
  • Develop, implement, and manage care plans to address clinical and social needs;
  • Respond to alerts related to patient vitals and provide appropriate clinical guidance;
  • Deliver patient education and support through video, phone, and asynchronous communication;
  • Coordinate care with physicians and serve as a liaison between providers and patients;
  • Assist patients in building healthier habits through personalized coaching and support;
  • Maintain accurate documentation within our platform.

Required Qualifications

  • Active and valid registered nurse (RN) license in the state of Illinois;
  • Bachelor’s degree in nursing or a closely related healthcare discipline;
  • Minimum of 3 years of experience in gastroenterology (GI) care;
  • Compact state license;
  • Strong customer service and communication skills;
  • Ability to work independently in a remote, technology-driven environment.

Preferred Qualifications

  • Experience in case management;
  • Bilingual or multilingual proficiency.
