Job description

Ohel is seeking a Peer Specialist for a dynamic field-based care transition team. This role will provide support, advocacy, and skill-building to foster independence and mental well-being. The Peer Specialist will have lived experience utilizing mental health and/or substance abuse services (SUD).

Responsibilities:

Engage participants in transition planning and needs assessment.
Conduct individual sessions focusing on prevention and independent living skills.
Provide emotional and practical support during community transition.
Resolve clinical issues impacting housing stability.
Foster collaborative relationships with community providers.
Advocate for participants' personal goals and address barriers.
Provide crisis intervention and support services as needed.

Qualifications:

High School Diploma or equivalent
New York State Certified Peer Specialist required
Experience in advocacy and behavioral health services preferred
Ability to use personal story for empowerment and recovery.

The ideal candidate is a self-starter who can work independently as well as within a team and has excellent follow up skills. This full-time position is located in Brooklyn, offers a hybrid schedule, with inpatient, home, and community visits required. Position includes a competitive benefits package.

Salary: $45,000
