Job description

Ohel is seeking a Peer Specialist for a dynamic field-based care transition team. This role will provide support, advocacy, and skill-building to foster independence and mental well-being. The Peer Specialist will have lived experience utilizing mental health and/or substance abuse services (SUD).

Responsibilities:

  • Engage participants in transition planning and needs assessment.
  • Conduct individual sessions focusing on prevention and independent living skills.
  • Provide emotional and practical support during community transition.
  • Resolve clinical issues impacting housing stability.
  • Foster collaborative relationships with community providers.
  • Advocate for participants' personal goals and address barriers.
  • Provide crisis intervention and support services as needed.

Qualifications:

  • High School Diploma or equivalent
  • New York State Certified Peer Specialist required
  • Experience in advocacy and behavioral health services preferred
  • Ability to use personal story for empowerment and recovery.

The ideal candidate is a self-starter who can work independently as well as within a team and has excellent follow up skills. This full-time position is located in Brooklyn, offers a hybrid schedule, with inpatient, home, and community visits required. Position includes a competitive benefits package.  

Salary: $45,000
