The referenced pay range represents the minimum and maximum compensation for this job. Responsibilities:

GENERAL PURPOSE
The CARE Case Manager provides strengths-based assessment, intervention, and coordination of services to students experiencing academic, personal, and/or medical difficulties to assist them in removing barriers to success and to enhance their holistic well-being. The Case Manager exercises independent judgment while coordinating with the Director of CARE, other faculty and staff members, and parents or community members when appropriate, to develop supportive plans for students in need of resources. This position also serves as a liaison between the Office of the Dean of Students (ODOS) and other campus departments to support student well-being, collect information about student referrals, and implement programs for various constituents in the university community. Key partnership and support for ODOS teams and events. The Case Manager assists the Director with oversight and administration of the CARE Network and serves as a team member in the Office of the Dean of Students. Occasional evening and weekend hours.

ESSENTIAL FUNCTIONS

CARE Student Case Management

  • Maintain a caseload of CARE referrals for students in need of additional support.
  • Conduct initial review of CARE referrals using the process of gathering data, analyzing through the NABITA rubric and implementing appropriate interventions.
  • Receive referrals from the Behavioral Intervention Team (BIT) to provide case management services and deliver recommended interventions.
  • Schedule and meet with student, assess student safety and needs holistically, coordinate action plan, referrals, and support including follow up appointments as necessary.
  • Communicate with referrers and provide updates, and collaborate with appropriate staff, offices, and resources.
  • Closely manages high risk cases including concerns such as suicidal ideation and/or attempt, substance use, serious mental health, and other behavioral issues.
  • Document notes in electronic record keeping system (Advocate)
  • Monitor and reassess utilization, completion, and efficacy of interventions, and amend action plan as needed.
  • Co-facilitate CARE Team and Student Support Network meetings under the leadership of the Director.
  • Coordinate with campus and community resources.
  • Manage CARE Network in the absence of the Director.
  • Working closely with the Director, provide leadership to the campus regarding issues of student support.

Programming and Marketing

  • Create and facilitate trainings, workshops, and presentations pertaining to student support and well-being
  • Represent CARE throughout the year at Resource Fairs, Orientation, Accepted Students Days, and more
  • Through outreach and one on one support, connect with faculty and staff strengthen recognizing and supporting students in need
  • Create, research, develop, and implement marketing outreach initiatives that contribute to better understanding and usage of CARE and case management services order promotional materials related to CARE.

CARE Network Administrative Tasks

  • Working with other CARE staff, maintain online case management system and track CARE Network referrals, documenting pertinent information while maintaining privacy and respect to sensitive information.
  • Assist with maintaining the CARE Resource Center online database and the CARE website.
  • Assist the Director with planning monthly Student Support Network meetings.
  • Assist the Director with collecting, interpreting, and reporting statistical data related to CARE Network referrals.

Regularly participate in

  • CARE staff meetings, ODOS and other departmental meetings. Behavioral Intervention Team meetings, and monthly Student Support Network Meetings
  • ODOS and University wide committees related to job responsibilities.
  • Regional, state and national professional organizations pertinent to the field
  • Assist ODOS offices with events and programs throughout the year, occasional evenings and weekends

MINIMUM EDUCATION & EXPERIENCE

  • Bachelor’s degree required.
  • Master's degree in higher education administration, student affairs, counseling, or related field preferred.
  • 2 years of experience in college or counseling setting or related experience, or equivalent combination of education and experience required.
  • Experience supporting students with mental health presentations, various disability statuses, and/or diverse identities of race, class, gender, sexual orientation, or nationality preferred.


KNOWLEDGE, SKILLS AND ABILITIES

  • Knowledge of higher education systems and related software and technology; effective communicator both written and verbal, handling sensitive and confidential information; ability to manage multiple projects simultaneously with meticulous attention to detail; ability to work independently while exercising good judgment; proven ability to develop and maintain effective working relationships in a complex environment required.
  • Knowledge of restorative justice and trauma informed practices preferred.
