2Life owns and operates over 1,500 apartments, 94% of which are affordable, located on campuses in Brighton, Brookline, Newton, Framingham, and Devens, Massachusetts. Another 500+ apartments are in development in Devens, Lynn, Mattapan, Newton, and Waltham, Massachusetts. We anticipate continued growth in the size of our portfolio, the geography in which we operate, and the diversity of our residents in terms of race, ethnicity, income levels, and need for supportive services. For example, in 2025 we will add a new community, Opus Newton from 2Life Communities, that creates an exciting living option for middle-income seniors.



GENERAL SUMMARY


The Opus Care Navigator (OCN) is a role for an experienced elder care professional to join a dynamic new team within a trusted organization where older adults get the personalized support they need to age with dignity, respect and purpose.


ESSENTIAL JOB FUNCTIONS


  • Collaborate with the Opus Newton Director of Health and Wellness to create a comprehensive healthcare and wellness program. This program should address all dimensions of wellness to foster healthy aging for residents within the community.

  • Manage a caseload of residents; responsible for ensuring care that supports (residents’) desired health and wellness outcomes

  • Assess residents upon move-in and annually  thereafter to determine their strengths, interests and needs

  • Educate residents (and where appropriate and desired, their families/care partners) about options and resources 

  • In partnership with residents, develop person-centered wellness plans, addressing their identified needs and prioritizing what matters most to them 

  • Document wellness plans and case management activities 

  • Follow up on referrals and monitor service delivery for quality and effectiveness, and to ensure needs are met

  • Build relationships with community service providers and other elder-care professionals

  • Participate in transitions-of-care meetings, and advocate for appropriate and ongoing medical care and social services for residents who request that support

  • Work to support 2Life wellness programs; participate in related activities as directed by the Director of Director of Care and Support Services


DUTIES AND RESPONSIBILITIES


  • Facilitate resident and family/care team meetings and relationships

  • Collaborate with the Executive Director , Chief of Community Life, Director of Care and Support Services, and other members of the Nahanton Campus team on processes, protocols, and resident communications

  • Develop, lead and/or oversee behavioral health and wellness initiatives in collaboration with other key team members

  • Provide brief, supportive counseling and crisis intervention as needed to residents and their care partners, referring to outside community provider/resources as appropriate

  • In consultation and collaboration with the Director of Care and Support Services, ensure all at-risk residents’ issues are identified and addressed

  • Maintain required professional licensure and certification

  • Other duties as assigned


PREPARATION, KNOWLEDGE, SKILLS & ABILITIES


  • Masters Degree in Social Work (MSW) plus 3+ years professional experience working with older adults in a community or medical setting preferred

  • Massachusetts licensure, LICSW

  • Aging Life Care Manager certification, or willingness to attain this within the first year of employment

  • Excellent communication, problem-solving, and organizational skills

  • Collaborative work style; enjoy working as part of a team

  • Must be flexible and able to work well independently

  • Proficiency with Google Workspace; familiarity with Salesforce desired


SUPERVISORY RESPONSIBILITY


  • No direct supervisory responsibilities at this time; may supervise a Social Work Intern in year 2


WORKING CONDITIONS/PHYSICAL DEMANDS


  • Normal office environment

  • Ability to walk throughout the building

  • Schedule based on the needs of the organization
