2Life owns and operates over 1,500 apartments, 94% of which are affordable, located on campuses in Brighton, Brookline, Newton, Framingham, and Devens, Massachusetts. Another 500+ apartments are in development in Devens, Lynn, Mattapan, Newton, and Waltham, Massachusetts. We anticipate continued growth in the size of our portfolio, the geography in which we operate, and the diversity of our residents in terms of race, ethnicity, income levels, and need for supportive services. For example, in 2025 we will add a new community, Opus Newton from 2Life Communities, that creates an exciting living option for middle-income seniors.


GENERAL SUMMARY


Experienced and accomplished Registered Nurse (RN) sought for the Director of Care and Support Services (DCCS) position. This individual should possess extensive care management and supervisory skills and be enthusiastic about joining a vibrant new team within a trusted organization dedicated to providing person-centered support to older adults, enabling them to live with dignity, respect and purpose. 


ESSENTIAL JOB FUNCTIONS


  • Oversee the Health and Wellness Program at Opus Newton Nahanton Campus which includes Care Navigation, Resident assessment and wellness planning, Social Day Program, Health and Care related contracted providers including the on site Medical Practice, approved personal care agency(s), and other providers

  • Work closely with the onsite Medical Practice to onboard residents into the practice and oversee the NP’s Care Navigation efforts

  • Lead the Interdisciplinary Team meetings 

  • Manage a caseload of residents, responsible for ensuring care that supports (residents’) desired health and wellness outcomes

  • Coordinate and manage resident care, acting as a liaison between residents, healthcare and other providers, both contracted by Opus and outside providers 

  • Assess and determine resident health and wellness needs using a comprehensive multifaceted tool 

  • Develop individualized wellness plan based on the assessment and make necessary revisions as resident status and care needs change

  • Document wellness plans and case management activities

  • Ensure all at-risk residents’ issues are identified, addressed, and monitored by resident services staff and reported to outside agencies as required. 

  • Educate residents about their health conditions and self-management strategies

  • Interact with internal and external health care providers to facilitate resident wellness, including pre and post discharge services around transitions of care

  • Identify and respond to discharge planning needs around transitions of care

  • Supervise direct and indirect reports, advise on high-risk cases, and identify and provide or arrange for staff training needs

  • Complete yearly performance reviews for direct reports 


OTHER DUTIES AND RESPONSIBILITIES


  • Take lead on outside health and wellness provider relationships, contracts and renewals

  • Hold wellness nursing hours in the Wellness Center to provide limited nursing functions including assessing vital signs, answer questions related to disease management and medications, and educate residents how to report results to their primary care provider

  • Maintain required professional licensure and certification

  • Department  budget responsibilities

  • Work to support 2Life wellness programs; participate in related activities as directed by the Executive Director


PREPARATION, KNOWLEDGE, SKILLS & ABILITIES


  • Active valid Massachusetts RN license

  • Bachelors of Science in Nursing (BSN) from an accredited school of nursing; Masters preferred 

  • Active MA CPR and First Aid certification

  • At least 2 years of direct care nursing experience, preferably in a healthcare or homecare setting 

  • At least 3 years of management experience including supervision in a senior living community, senior care community,  or 2 years experience as a Life Care Planner 

  • Case Management and Care Coordination experience a plus

  • Aging Life Care Manager Certification preferred

  • Strong organizational skills and ability to set priorities. Ability to compile data from concurrent and retrospective record review to identity trends and opportunities for proactive intervention(s)

  • Excellent written and verbal communication skills

  • Proficiency with Google Workspace; familiarity with Salesforce desired


SUPERVISORY RESPONSIBILITY


  • Direct supervision of the Social Work Opus Care Navigator and Social Day Program Manager and other staff as needed

  • Liaison with onsite Medical Practice

  • Responsible for oversight of contracted community Health and Wellness Providers

  • Normal office environment

  • Ability to walk throughout the building

  • Schedule based on the needs of the organization

  • Willingness to work a rotating schedule as manager on duty.
