Responsibilities
Program Leadership:
- Lead the Connect For Life program, utilizing Wider Circle's developed curriculum to build trust and promote health.
- This means leading events, virtual and in person and creating a community of connected members. Members should, under your leadership, connect with each other, attend events frequently and support each other.
- As a community engagement lead, you will be seen as a lead to others in the organization supporting and leading your peers as needed by your manager.
Event Planning and Hosting:
- Strategically plan, and host virtual and in-person events to foster relationships among members and contribute to positive wellness outcomes.
- Planning includes venue search, prep, and outreach to members to confirm their attendance to your events.
- Assisting peers and others with venue support and event planning as needed.
Member Engagement:
- Proactively contact members, ensuring participation in Chapter events and various group activities, conducting one-on-one home visits, and facilitating monthly engagement.
- Meet monthly requirements as it relates to member engagement, member attendance and helping members solve care needs.
Documentation:
- Record member interactions ( all member outreach, attendance and attempts to contact members) for effective tracking and reporting, through wider circles online digital systems, in a timely manner.
Leadership Identification:
- Identify natural leaders within the community and invite them to serve as Wider Circle Ambassadors, enhancing member connections and event coordination. Ambassadors help support and grow communities, while encouraging a sense of purpose with our members.
Prospective Member Outreach:
- Connect with potential members, developing personalized plans to enroll them in the Connect For Life program. This could be via phone, 1:1 in person engagement or at events.
Liaison and Partnerships:
- Serve as a liaison, connecting members to their health plans and to each other. Support members in needs as it relates to helping them with health care services, doctor appointments or assisting them in closing care gaps.
- Establish partnerships with local community-based organizations to broaden the range of services available to our members. CEL Not CES
Accountability:
- Hold yourself accountable for achieving membership engagement and growth goals, monitoring success through metrics such as attendance, feedback, and other relevant data.
- Manage your time and ability to track and report out on hours worked in a remote environment.
- Support your Area Manager is assisting CES in their role, this can include training, shadowing or providing support to associates needing assistance.
- Ability to support your membership and hit required standards with little to supervision.
Additional Responsibilities: These may be required based on your location and the company's needs.
- Home Canvassing: Conduct in-home visits to, meet with members that you are unable to reach or to meet with them to review member needs and encouragement to get them to events.
Physical Demands:
To successfully fulfill this role, you must be able to perform the following essential functions:
- Safely and consistently drive to public places up to 30 miles away from your home.
- Frequently carry up to 30 pounds of supplies.
- Comfortably stand and speak publicly, projecting your voice in indoor and outdoor spaces.
- Sit, stand, and walk routinely during events to interact effectively with members.
- See and hear well enough to interact with members in person or by phone and respond to member questions.
- Use a computer to view, enter, and update information in different systems.
- Effectively communicate with members in person or virtually.
Requirements
- A genuine interest in working with Medicaid/Medicare/DSNP populations and a commitment to building strong, supportive communities.
- Strong verbal and written communication skills to effectively engage with diverse community members.
- Team Player: Ability to collaborate with team members and contribute to a positive team environment. As well as be seen by others as a leader and support when the manager is not around.
- Proactive approach to outreach, with the ability to take the lead on community engagement initiatives.
- Adaptability to a dynamic work environment and a willingness to learn and grow in the role.
- 3+ years of relevant community outreach, teaching, facilitation, or volunteer experience; healthcare experience preferred.
- Associate degree; a bachelor's is preferred. Work experience of 5+ years in lieu of a degree can be considered.
- Experience creating a community or supporting upwards of 500 members in a community-based program.
- 1-2 Years equivalent experience or Associate's degree preferred
- Exceptional organizational and prioritization skills, with the ability to follow through on commitments.
- Proficiency in computer skills and the ability to navigate web-based and app-based systems.
- Ability to work independently in a remote work environment with little oversight.
- Reliable transportation and comfort with traveling to members’ homes, willing to commute 20-50 miles.
- Flexible schedule to accommodate work outside regular business hours when necessary.
- Commitment to a drug-free workplace and readiness for pre-employment substance abuse
- testing and background checks.
