Case Manager - Noble and surrounding counties.

CareStar, Inc. is currently seeking a Case Manager in the Specialized Recovery Services (SRS) and Ohio Home Care (OHC) Programs to conduct eligibility evaluations for individuals applying for specific programs and creates a person-centered service plan. He/She will assist the participant with access to and coordination of services to enable the individual to receive a range of appropriate services in a planned, coordinated, efficient and effective manner; promotes and supports the participant’s preferences, values, and the right to self-determination.

THE POSITION

CareStar Is Looking For Motivated Individuals Who/to

  • Maintains knowledge of Medicaid, of the behavioral health system and of community resources.
  • Conducts face-to-face eligibility evaluations, including but not limited to, administration of the Adult Needs and Strengths Assessments; verification that the individual resides in a program approved environment; and verification of the individual's qualifying behavioral health diagnosis; other eligibility criteria as described in program specific guidelines ensuring that such evaluations are completed in accordance with legal, program, contract and/or CareStar requirements.
  • Develops, monitors, and updates individual's person-centered care plans to ensure the individual's needs, preferences, health, and welfare.
  • Coordinates and authorizes all specialized recovery services program services.
  • Assists individuals with connections to needed resources, including Medicaid services, medical, social, educational, and other resources, regardless of funding source.
  • Approves funds for utilization of available services, while meeting all qualifications and standards as specified by the program; develops annual cost budgets.
  • Functions as the team leader in the development and implementation of the consumer’s care plan, including the authorization of the amount, scope and duration of services.
  • Adheres to the CareStar Rule in performance of job responsibilities.
  • Understands and complies with CareStar Policies and Procedures.
  • Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
  • Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

Qualifications And Education Requirements

  • Minimum Bachelor's Degree in social work, counseling, psychology, or a related field; Licensed Social Worker (LSW) and Registered Nurse (RN) preferred.
  • 12 months experience in home and community based services within the past 5 years.
  • Minimum of one-year post degree experience working with individuals with diagnosed chronic conditions and/or minimum of three years’ post degree experience working with individuals with severe and persistent mental illness.
  • Knowledge of issues affecting people with severe and persistent mental illness and community-based interventions/resources for those individuals.

This position is open in Noble County and is responsible for covering the surrounding counties.
