Care Liaison Advisor

Xyla Community Services (part of Acacium Group)

Hybrid model with regular on site work at James Paget Hospital (NR31 6LA)

Full Time with regular weekend work

6 Months Fixed Term Contract

£25,000 per annum

We are looking for a Care Liaison advisor to join our team, where you will play a vital role in our company's success. In this role, you will closely collaborate with hospital discharge teams and care homes to source the best care packages for our clients.

This position offers a unique opportunity to work towards quick and safe discharge plans, supporting families during challenging times.

Every day you will…

  • Communicate through regular touch points both over the phone and in person, updating system notes to accelerate patient discharge
  • Drive customer satisfaction and deliver results through execution of effective care placements
  • Act as the link between discharge teams, local authority, clients, their families and healthcare providers to facilitate discharge and ensure a seamlessly coordinated service
  • Organise caseloads in priority order having fully understood patient needs
