JOB DESCRIPTION

The Physical Therapist will provide evaluation and treatment for conditions within their scope of practice.

JOB QUALIFICATIONS

EDUCATION

(For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.)
Required Education: Bachelor of Science, Masters, or Doctorate degree in Physical Therapy is required.

EXPERIENCE
Preferred/Optional Experience: Experience in physical therapy is preferred.

CERTIFICATIONS/LICENSES

The following licensure(s), certification(s), registration(s), etc., are required for this position, which must be maintained in good standing:
Minimum Required: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire. Physical Therapist license awarded by the State of Wisconsin

Preferred/Optional: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
