Job description

The OnTrackNY Program is a Coordinated Specialty Care (CSC) treatment program at Community Health and Behavioral Services serving adolescents and young adults who are impacted by unexpected changes in their thinking and perceptions. Our mission is to empower young people to make meaning of their experiences and to pursue their goals for school, work and personal relationships. OnTrackNY team staff receive expert training and technical assistance in the CSC model and protected time for professional development through the statewide OnTrackNY Initiative at the Center for Practice Innovations at Columbia Psychiatry and New York State Research Foundation for Mental Hygiene.

 

The Team Leader role is a full-time position. The Team Leader leads a small multidisciplinary team, supports outreach and recruitment activities and provides coordinated and assertive treatment to enrolled participants and their families. 

 

CORE RESPONSIBILITIEs

    • Fosters and models principles that underlie the model (recovery, shared decision making/duty to care-dignity of risk, person centered, cultural humility).
    • Ensures all team members are kept informed of and collaborate on issues involving participants and their families.
    • Provides overall supervision to the team and is responsible for allocation of staff time.
    • Ensures coverage of 24/7 call schedule and access to team.
    • Ensures team members can deliver services in the community (off-site).
    • Monitors caseload size and coordinates program intakes and discharges.
    • Facilitates weekly team meetings and provides training and individual supervision to team Primary Clinicians, Supported Employment and Education Specialist, and Peer Specialist; and provides administrative supervision to the Prescriber and Nurse.
    • Serves as liaison between team and Community Health & Behavioral Services to ensure adherence to the coordinated specialty care model in the contest of agency name protocols and policies.
    • Ensures staff attendance at required OnTrack NY trainings and consultation calls, and completion of additional OnTrackNY related training modules in the Center for Practice Innovations’ Learning Management System (LMS)
    • Establishing warm and collaborative relationships with community partners who might refer or provide ongoing support to young people and families.
    • Spreading awareness about OnTrackNY services through community education and outreach activities that emphasize OnTrackNY’s uniquely flexible and youth-friendly approach.
    • Collaborate with OnTrackNY team members to set new community-based goals focused on reducing inequities and barriers to accessing specialty care.
    • Identifying and creating new resources to support diverse young people and families seeking or receiving services at OnTrackNY.
    • Serves as the Primary Clinician (PC) for a subset of program participants, as determined by census need. PC duties may include, but not be limited to:
      • Serves as the point person for the participant and their family;
      • Provides services in the community (off-site) as needed based on participant and family preferences.
      • Conducts needs assessments;
      • Engages in safety planning and wellness management planning with participants;
      • Connects participants and families with the services and supports they need;
      • Provides behavioral interventions and take an especially active role in the family support domain.
      • Conducts clinical assessment to determine the participant’s level of social functioning and substance use via a shared decision making (SDM) framework
      • Provides Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psycho-education.
      • Provides psychoeducation regarding first episode psychosis (FEP) and the goals of the OnTrackNY program;
      • Provides supportive therapy;
      • Develops and modifies treatment using a shared decision-making process
      • Works with participants and family for transition from the team to other services and support, and ensures stable links at discharge.

     

    REQUIRED QUALIFICATIONS 

    • Licensed Masters or Ph.D. in an appropriate field (social work, psychology, mental health counseling).
    • Requires occasional travel for client home visits and community-based services - valid NYS Drivers License required. 

     

    PREFERRED QUALIFICATIONS

    • 5+ years training and experience working with adolescents and/or young adults with serious mental illness.
    • Experience in a community-based mental health services.
