Job Title: Care Coordinator
Location:
Remote (PST Time Zone)
Salary Range:
up to 2500 USD

Work Schedule:
Full-time (40 hrs/week) with phone coverage needed between 6:30 AM to 7:00 PM (PST), Monday to Friday; weekend availability preferred but not required

Position Overview:

We are looking for a warm, professional, and highly dependable Care Coordinator to serve as the first point of contact for our patients. In this role, you'll manage phone communication, process referrals, coordinate care across clinics, and provide compassionate, knowledgeable support to ensure every patients journey is smooth and respectful from start to finish.

Key Responsibilities:

  • Handle inbound and outbound patient calls with empathy and professionalism.
  • Process new referrals and follow up with patients and providers.
  • Coordinate scheduling across multiple clinics.
  • Maintain accurate and up-to-date patient records.
  • Clearly explain key information such as insurance, policies, and care plans.
  • Operate across multiple systems, including EMR platforms, Zoom, and Notion.
  • Communicate in a trauma-informed, inclusive, and supportive manner.


Qualifications:

  • Fluent spoken and written English with minimal accent.
  • Excellent communication and customer service skills.
  • Strong attention to detail and ability to follow structured processes.
  • Empathetic, patient, and professional in all interactions.
  • Reliable internet connection and quiet work environment.
  • Comfortable learning and using digital tools and software.
  • Able to work independently in a remote setting.
  • Availability to work during core PST hours as part of a rotating team.


Nice-to-Haves:

  • Prior experience in healthcare, medical admin, or call center roles.
  • Familiarity with EMR software and clinical coordination tools.
  • Female candidates are preferred (due to patient demographic needs in pelvic health).
  • Experience supporting LGBTQ+ and non-binary patients.
  • Sales mindset, comfortable communicating the clinics value to new patients.
