The LPN at Bristol Health will be an integral part of the Med/Surg Nursing team adding value to the patient care paradigm. Working as part of a collaborative patient care team made up of RN’s, LPN’s and PCA’s we are focused on delivering outstanding patient care in our Magnet facility.
The LPN as part of the nursing team will contribute to the nursing and patient care by:
Delivering care, collecting, reporting, and recording data and/or assessments for accuracy and timeliness with RN oversight.
Participating in the development of the patient's care plan.
Participating in team decisions for patient care
Utilizing delegation and autonomous skills within the LPN scope of practice for direct patient care. The LPN will establish individualized care delivery based on priority of needs and pre-established goals and plans with RN.
Demonstrating behaviors which support an environment conducive to safety, health and positive culture.
Accurately documenting nursing interventions, data collection and responses to care and communicating them to appropriate members of the healthcare team.
Working collaboratively with the RN and PCA to perform patient related activities, medication administration, therapeutic treatments which all contribute to safe and efficient delivery of patient care.
Observing or identifying conditions or changes in patient status and reporting in a timely manner to RN or medical team.
Supporting and participating in unit and hospital quality initiatives.
Demonstrating accountability for professional competency and practice.
Demonstrating knowledge and critical thinking to promote optimal patient care.
REQUIREMENTS:
• State of CT LPN License
• Good oral and written communication and interpersonal skills.
• CPR certified.
Experience:
Prefer one year of experience
