Description

Mountain View Hospital is looking for a Chronic Care Management (CCM) / Licensed Practical Nurse (LPN) / Registered Nurse (RN) to join our team!

JOB SUMMARY: The LPN/RN provides direct and indirect patient care in the clinic setting.  He/she communicates with providers/nurse manager/co-workers as appropriate, about changes in patient’s clinical condition including results of diagnostic evaluation and changes in patient condition or symptomatology.  The nurse will be able to respond quickly and accurately to changes in condition or response to treatment, and avail his or herself of all training, skills and ability commonly developed through attainment of licensure in their field.  The nurse may also have inventory/ordering responsibilities in addition to clinical responsibilities, and will expected to cross train in additional areas of clinic responsibilities including laboratory rapid assays, reception and radiographic studies.

DUTIES AND RESPONSIBILITIES:

Chronic Care Management (CCM)

  1. Solid understanding of the healthcare continuum
  2. Demonstrate sound judgment, patience and maintain a professional demeanor at all time.

  5. Computer skills: Word, Excel, Outlook, Electronic medical records software

  7. Ability to utilize data and reports to develop actionable interventions and measurable goals
  8. Creativity, complex problem analysis and decision making
  9. Ability to work varied shifts
  10. Stay current on CCM changes and requirements from CMS
  11. Other tasks as assigned by the supervisor or physicians
  12. Manage gap reports
  13. Identify special needs and establish a care plan to follow when beginning to coordinate healthcare services.
  14. Act as a liaison between our provider and the patient when coordinating community resources, access to specialty services, follow-up healthcare, etc.
  15. Follow-up and help complete identified referrals. Coordinate with specialists and help to integrate recommendations from our provider. Identify barriers for lack of follow through or potential barriers to community resources.
  16. Conduct intermittent telephone consultations with high-risk clients to verify status of follow-up and determine if qualifying triggers have been satisfactorily resolved.
  17. Schedule complex clients for follow-up with provider and attend office visits if determined appropriate.
  18. Serve as the primary point of contact for patients considered “high risk.”
  19. Attend executive management meetings if indicated by invitation.
  20. Follow HIPAA guidelines when coordinating care

LPN/RN

  1. Work under direct supervision of MD, PA-C or NP to provide excellent patient care to all clients that come to IF Clinic.
  2. Performs a head-to-toe assessment on all patients and performs reassessments as per policy.  This includes all age appropriate care including: pediatric, geriatric, and the general patient population. Assist in minor office procedures and appropriate medical screening exams.
  3. Adequately assesses and reassesses pain.  Utilizes appropriate pain management techniques.  Educates the patient and family regarding pain management.
  4. Performs waived testing (point-of-care testing) per clinical laboratory’s and the patient care unit’s policies and procedures as needed when the lab tech is unavailable and performs tests in a timely manner and interpret results of waived test.
  5. Is knowledgeable of medications and their correct administration based on age of the patient and their clinical condition using 5 medication rights.
  6. Interacts professionally and treats patients and their families with respect and dignity.  Involves patient/family in the formation of the plan of care. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Functions as a liaison between administration, patients, physicians and other healthcare providers.
  7. Consults other departments as appropriate for an interdisciplinary approach to the patient’s needs.
  8. Communicates appropriately and clearly to office manager, co-workers and physicians.
  9. Documentation meets current standards and policies.
  10. Manages and operates equipment safely and correctly.
  11. Is flexible, organized and functions under stressful situations.
  12. Completes annual education requirements, adheres to infection control, safety guidelines, ethical business practices, and confidentiality policies.
  13. Wears identification while on duty, uses computerized punch time system correctly.
  14. Attends at least 75% of staff meetings annually.
  15. Represents the organization in a positive and professional manner.
  16. Actively participates in performance improvement and continuous quality improvement (CQI) activities.

Qualifications

Education/Certification: LPN/RN degree with associated IV therapy certification and drug administration certification within 60 days of hire, along with BLS/ACLS/PALS certifications as above.  The LPN/RN may substitute PEARS instead of PALS. 

Experience: 6 months required experience, 2-5 yrs clinical experience preferred  

Equipment/Technology: Basic computer knowledge.  

Language/Communication: Be able to read and write in English.

Mathematical Skills: Must be able to accurately calculate drug dosing and drip rates for IV therapy.

Mental Capabilities: Can concentrate for long periods of time in order to complete assigned tasks with a better than average attention span in order to listen, read and remember verbal and written instructions.  Can understand input from the supervisor and/or manager concerning errors and instructions.  

Interpersonal: Ability to work well with others.

Performance: Must be able to handle conflict issues with both patients and staff.
