Description

This is a direct care position in our transitional housing program serving persons with medical, mental illness, or substance use disorders. The Client Care Coordinator is responsible for ensuring client safety, providing direct services, and maintaining site safety during all shift hours. Crisis de-escalation and support during such incidents is part of CCC responsibilities. Collaborating with housing team in support of client goals, as well as program needs is included in task responsibility. CCCs support full housing team in obtaining and filing all required documentation. 

We are looking for staffing from Wednesday to Sunday - 8am to 4pm or 9am to 5pm, full-time/part time hours. 

 

Responsibilities: 

- ensure client safety providing direct services and maintaining site safety during all shifts. 

-Monitor self-administration of medication according to policy/procedure (not required for all clients, only as needed basis). 

-Assist clients with managing their activities of daily living (ADL) skills by providing daily living skills education. 

-Oversee client activities, run socialization groups

-Monitor surveillance camera for safety and site security as needed. 

-Provide client transportation to program related activities as needed. 

-Accompany clients to appointments as needed. 

-Crisis de-escalation and crisis management. 

-service delivery per program scope and documentation of completed services, as well as documentation of safety rounds.

-support case management staff and overall program with task completion and or client follow up.

Qualifications

-HSD/GED with relevant experience required.

-valid clean driver's license required

-Experience working with homeless, MICA, and or substance abuse populations needed. 

-Ability to establish workload priorities and balance diverse project needs. 

-Must have exceptional communication skills both orally and in writing regarding complex and sensitive issue
