Description

POSITION SUMMARY:  The Triage Nurse will provide professional nursing evaluations, prioritize treatments according to the urgency of need, and initiate medical care to participants. Coordination of care will occur with collaboration from Rocky Mountain Health Care Services (RMHCS) program provider. The schedule available is Monday and Tuesday 5:30 PM to 8:30 AM, and is subject to change. An additional shift differential is also offered on top of the pay scale listed.

RESPONSIBILITIES AND DUTIES:

ESSENTIAL JOB FUNCTIONS:

  • Interviews participant and/or MDPOA to obtain medical histories and to make an accurate evaluation.
  • Reviews chart for pertinent information
  • Collaborates with provider to assist with appropriate treatment measure
  • Assign triage priority based on observations and available information.
  • Utilize Clear Triage for every triage visit copy/paste note into participant chart
  • Initiates therapeutic measures as indicated by standing protocols and direction from RMHCS program provider.
  • Calls for crisis intervention if appropriate.
  • Evaluate if participant is necessary for Same Day Clinic visit with provider
  • Communicates and acts as liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity.
  • Coordinates the needs of the patient with supporting departments and assists in solving problems related to the welfare of participant and staff.
  • Charts concisely, descriptively, and legibly indicating patient’s progress or lack of progress immediately after triage completed or at earliest convenience and always same day of triage.
  • Performs nursing procedures safely and efficiently.
  • Demonstrates understanding of and adheres to the infection control, sanitation and patient safety and confidentiality protocols.
  • Demonstrates compliance with exiting standards and policies.
  • Participates in mandatory in-services.
  • Assist with administration of vaccinations when needed
  • Maintain monthly oxygen report and notify provider if orders do not exist or do not match (Day Center triage only).

Qualifications

  • Possess an active, Unencumbered Colorado Nurse License
  • Graduate of an accredited Nursing Program and licensed as a professional in the State of Colorado is required
  • Two years of health care experience in hospital, long-term or home health care setting required
  • One year of experience with a frail or elderly population is preferred
  • Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers
  • Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records
  • Knowledge of medical equipment and instruments 
  • Knowledge of common safety hazards and precautions to establish a safe work environment
  • Skill in applying and modifying the principles, methods and techniques of practical nursing to provide on-going care for participants
  • Ability to perform phlebotomy functions.
