Primary Work Location:
Fairmont Respite is a tiny home community located near Fairmont Hospital. Cardea Health provides clinical services at Fairmont Respite, a 34 bed tiny home community that services individuals with clinical needs and provides housing navigation services. Cardea Health partners with Alameda County for the Homeless agency and the housing agency Five Keys to provided tailored services to unhoused individuals working to transition to permanent housing.
Job Summary:
The housing coordinator will assist clinical staff and shelter residents in the coordination of medical and behavioral health care and works alongside a diverse clinical team at a medically supported shelter program serving people experiencing homelessness who are recovering from recent hospitalization, injury, or illness. The housing coordinator will provide a variety of functions to support Cardea Health clinical programs at the Medically Supported Shelter. The ideal candidate is organized and motivated. The housing coordinator supports resource linkage and develops housing plans for individuals who have experienced chronic homelessness. The housing coordinator helps individuals navigate the complex housing system, identify the appropriate resources they will qualify for, and help them successfully exit the medically supported shelter program in a timely manner. The ideal candidate has experience with homeless or incarcerated populations. This is an excellent position for someone who is seeking to gain experience in a healthcare social service setting.
Job Duties & Responsibilities:
Completes screenings and assessments to identify housing needs and factors contributing to housing instability, including functional, environmental, employment, educational, and health information, as appropriate
Develops housing plans for each resident and ensures they are on track to successfully exit the program by or on their anticipated discharge date
Assists all shelter residents to become document ready for future permanent housing
Help connect residents to Housing Resource Centers (HRCs) and complete Coordinated Entry System (CES) assessments
Facilitates shelter referrals
Completes and maintains documentation of their work using the standard work developed by Cardea Health
Develops and maintains collaborative relationships with providers, facilities, community resources, etc
Serves as a strong advocate to help shelter residents obtain the housing resources they are eligible for
Maintains confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA)
Practices harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect
Other duties and responsibilities as required
Minimum Required Skills & Qualifications:
High School Diploma or equivalent
Must be authorized to live and work in the U.S.
Must have personal, reliable transportation to and from work sites and shifts
Must be 18 years of age
Self-directed and able to work with minimal supervision
Detailed oriented and highly organized
Compliance with all company policies
Must provide proof of COVID vaccination or willing to be vaccinated
Prior experience with homeless or incarcerated populations is desired
Proficiency in Microsoft Outlook, Word and Excel is required

Preferred Skills & Qualifications:
Bachelor's or associate degree in a relevant field

This position will require traveling to other office locations or off-site meetings as required by the job
Compensation and Benefits:
Competitive Hourly Pay Rate: ($28-$33)
