The Role

We are seeking an experienced and passionate Director of Care Navigation to lead our patient support and care management efforts. This role is critical to the success of our clinical model, ensuring that every individual who enters our system feels supported, seen, and empowered.

You will oversee a national team of Care Navigators and support staff, and in time, build and manage a layer of care navigation team leaders. This role blends strategic leadership, operational rigor, and deep empathy for patients and caregivers navigating memory loss. You’ll be responsible for team development, performance, patient engagement outcomes, and continuous improvement of care coordination workflows.

The ideal candidate brings deep experience in dementia care, a proven ability to scale and manage multidisciplinary teams, and thrives in a fast-paced, tech-forward healthcare environment.

Key Responsibilities

Team Leadership & Strategy

  • Directly lead a growing team of Care Navigators and support staff, while preparing to scale the team structure by hiring and developing team leaders
  • Set clear goals and performance standards; foster a high-performing, mission-aligned team culture
  • Lead strategic planning and execution of care navigation processes in alignment with clinical and organizational priorities

Patient Experience & Care Coordination

  • Own the patient journey from first touchpoint through longitudinal engagement; ensure timely, consistent, and compassionate care navigation
  • Partner with clinical and operations leadership to identify opportunities to improve patient and caregiver engagement, retention, and satisfaction
  • Oversee complex patient escalations, transitions of care, and interdisciplinary coordination efforts

Operational Excellence

  • Optimize workflows, documentation standards, and tech-enabled task management across the Care Navigation function
  • Own and improve performance metrics such as check-in completion rates, care coordination timeliness, and navigation quality
  • Support integration with partner organizations and payors, ensuring SLAs and reporting needs are met

Cross-Functional Collaboration

  • Act as the strategic liaison between Care Navigation and Clinical, Tech, Product, and Growth
  • Support implementation of new programs or services that impact care coordination (e.g., group therapy, diagnostics, caregiver support)

About You

Must-Have Qualifications

  • 5+ years of experience managing care coordination, case management, or navigation teams, ideally within a healthcare, aging services, or digital health context
  • Demonstrated leadership in the Alzheimer’s disease, dementia, brain health, or geriatric care space — you understand the nuances of this population’s needs
  • Proven ability to lead in a fast-paced, dynamic organization (e.g., startups, healthcare innovation settings)
  • Experience using care coordination technology, EHRs, and/or telehealth platforms
  • Exceptional communication, people leadership, and change management skills
  • Strong operational mindset: you think in workflows, metrics, and systems—but always center the human experience

Preferred Qualifications

  • Advanced degree in Public Health, Social Work, Nursing, Healthcare Administration, Counseling, or related field
  • Familiarity with NCQA care management standards and best practices
  • Experience building or scaling programs across multiple states or regions
  • Familiarity with Medicare/Medicaid billing is a plus
  • Prior experience supporting family caregivers, behavior management strategies, or navigating community resources
