Job Summary : The Family Worker provides a variety of supportive services and case management activities for families participating in the Family Reception Center program in order to ensure child safety, well-being, and family stability.
Major Duties
Create and maintain linkages to important community resources and assist families in accessing these services such as medical and mental healthcare, housing, childcare, and public benefits.
Attend community-based and funder-provided training and informational sessions and share out knowledge and resources gained with staff and participants.
Accompany participants to designated meetings and appointments for educational or medical purposes, and/or to government and social service agencies to assist and encourage families in their progress towards meeting goals.
Assist the Case Planner to conduct home and office visits with families to monitor child safety and ensure family stability, where indicated.
Engage in crisis intervention activities as needed during periods of family instability, de-escalating conflict and ensuring safety.
Attend and participate in program staff meetings, as well as related agency training as appropriate.
Support Solution-Based Casework interventions with all families and assist with planning and scheduling Family Team Conferences.
Complete statistical/written reports in required databases (Connections and PROMIS) and write progress notes for contacts made with participants, including summaries as needed.
Perform other duties as assigned.

Must be able to travel and feel comfortable making home visits throughout Brooklyn.
Willingness to understand and be sensitive to varied cultures & backgrounds.
Must have a proven record of successfully navigating New York City entitlement programs.
Must work a maximum of two evenings per week.
Must work with participants from a strength-based, youth and family development approach.
