Job Description:
As an LPN you will be responsible for delivering direct nursing care to residents, assisting in daily medical management, and collaborating with the healthcare team to ensure the highest standard of care. You will have the opportunity to build relationships with residents and their families while ensuring a comfortable and supportive living environment.
Key Responsibilities:
Administer medications and treatments as prescribed by physicians.
Monitor and record vital signs, symptoms, and changes in patient conditions.
Assist in the development and implementation of care plans.
Provide direct patient care, including personal hygiene and mobility assistance.
Collaborate with registered nurses (RNs), physicians, and other team members in delivering care.
Respond to medical emergencies and provide first aid when necessary.
Maintain accurate and up-to-date medical records for residents.
Ensure compliance with facility policies and regulations, including infection control procedures.
Promote a positive and supportive environment for residents and staff alike.
Qualifications:
Valid LPN license in NY.
Current CPR and First Aid certification.
Previous experience in long-term care or a healthcare setting preferred, but not required.
Strong communication skills and the ability to work well with residents, families, and team members.
Compassionate, patient, and detail-oriented with a strong sense of responsibility.
Ability to work independently and as part of a team.
Strong time management and organizational skills.
