A DAY IN THE LIFE OF A COMMUNITY LIVING SPECIALIST In this position, you will provide person-centered care coordination with older Minnesotans and caregivers over the phone and in-person. You will be providing options counseling, advocacy, resource location assistance and education. You will research home and community-based service options and assist clients as they navigate systems and comply with specific protocols and documentation requirements.?
