Job description

Position Purpose: Supervises community and resource coordination activities to ensure members are connected to local resources and are being supported throughout care/service. Supervises day-to-day activities of community and resource coordination team members and issues related to members. Position will work remotely and will need to work CST hours, preferred to be seat ed in a CST state.

(Will be required to work 4, 10 hour days Friday through Monday. Schedule will be 8a-7p CST).

  • Monitors and reviews all community and resource coordination activities including member outreach and ensuring members are connected to local resources throughout care/service
  • Assigns community and resource coordination tasks to the team based on state and contractual requirements and member needs
  • Works with community and resource coordination and care management team to ensure effective, appropriate care of the member and addressing member needs
  • Ensures compliance with state and regulatory guidelines and standards
  • Educates and provides guidance and/or resources for community and resource coordination team to address member requests/needs
  • Evaluates community and resource coordination team’s performance and provides feedback regarding performance, goals, and career milestones
  • Provides coaching and guidance to community and resource coordination team to improve community resources, outreach services and connecting members to community resources to provide necessary care in a cost-effective manner
  • Assists with onboarding, hiring, and training community and resource coordination team members
  • Performs other duties as assigned
  • Complies with all policies and standards

Education/Experience: Requires a Bachelor's degree and 4+ years of related experience.

Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position.

Pay Range: $62,400.00 - $112,400.00 per year
