Job Type: Contract-to-Hire

Schedule: Full-time, Monday–Friday

Location: Remote (Must reside in Pacific or Mountain Time Zone)

Pay: $40–$41/hour

We are hiring a compassionate and experienced Registered Nurse (RN) Disease Manager with a focus in Behavioral Health to support our client on a federal TRICARE contract. This is a remote, contract-to-hire opportunity for RNs who are dedicated to improving mental health outcomes for TRICARE beneficiaries.

Key Responsibilities

Provide telephonic behavioral health disease management and patient support
Assess, plan and coordinate care for patients with chronic behavioral health conditions (e.g., depression, PTSD, anxiety)
Deliver education and interventions that promote mental wellness and self-management
Collaborate with providers and care teams to ensure continuity of care
Maintain detailed and timely documentation in electronic systems

Requirements

Current, active RN license in California (required)
Must reside in the Pacific or Mountain Time Zone
Minimum 2 years of recent RN experience in behavioral health/case management
Experience working with behavioral health populations (civilian or military)
Excellent communication, assessment, and follow-up skills
Comfortable working independently in a remote setting
