Title: Cardiovascular Quality Data Specialist - DH Quality Data - Full time/40 hours per week, 8 hour shifts. Remote
Summary: Works in all aspects of Cardiovascular Registry Functions in the Quality Data Department to include facilitating improvement in medical record documentation through extensive interaction with physicians, medical staff and nursing staff to ensure the documentation supports the quality and performance metrics of the HVI and the Clinical Data Registries. Supports the daily operations of the quality Data Department utilizing clinical knowledge and understanding of the Registry Products and Services.
Responsibilities:
1. Collect Clinical Data for Cardiovascular Data Registries.
2. Applies clinical knowledge and subject matter expertise to accurately collect a wide variety of clinical data from medical records with the designated time frames.
3. Concurrently provides education and collaborates with medical staff and hospital staff to ensure optimal documentation that will support the goals of the HVI.
4. Conducts follow-up reviews of clinical documentation to ensure that the issues discussed and clarified with medical and hospital staff have been recorded in the patient’s medical record.
5. Independently, accurately and completely perform validation of data prior to any data submissions.
6. Analyzes data for HVI and provides reports of Clinical Outcomes and Quality Metrics.
7. Accurately analyze aggregate reports of Clinical Registry Data.
8. Prepares & provides reports of clinical performance measures and statistics to the designated internal customers within the designated time frames.
9. Provides report of clinical performance measures to the designated external customers within the designated time frames.
10. Identifies and provides external comparisons for the Registry Quality and Performance metrics to HVI leaders as requested.
11. Performs internal quality audits to ensure integrity and accuracy of data.
12. Facilitates improvement in performance and medical record documentation as identified through the data registry analysis.
13. Identifies opportunities for Quality Improvement, and identifies trends through chart review and shares with internal customers.
14. Concurrently provides feedback to medical and hospital staff related to Registry specific requirements.
15.Provides education as appropriate when there are changes to the Data Registry elements or process changes.
16. Maintains positive, open communication with Physicians and Interdisciplinary care team members and provides physician specific feedback to designated members of medical staff.
17. Collaborates with IT, Physician experts and other staff to develop technology to streamline clinical data collection improving accuracy and optimizing automated processes.
18. Coordinates Meetings for the HVI specific to Data Registries and participates and collaborates to provide information for various cardiology meetings including M&M, Division of Cardiology, QPIC.
19. Prepares information for the Cardiology Service line meetings.
20. Organizes recurring meetings to review all adverse events, deaths, opportunities for improvement as identified through review of data.
21. Actively participates in HVI committee meetings to identify, share and develop action plans to improve patient care.
22. Maintains and tracks follow-up monitoring of action plans to improve patient care.
23. Other duties as assigned.
Other Information:
Required education/skills: Registered professional nurse with minimum of three years job-related experience in data registries, cardiology or performance improvement preferred; or Bachelor’s degree in Health related field with minimum of 3-years job-related experience in data registries, cardiology or performance improvement preferred.
Critical thinking skills, decisive judgement and ability to work with minimal supervision

Salary Range: $40.43 - $75.10 hourly (Pay per years of relative experience)
