About the role:

Accompany Health Pharmacy Technicians are a key part of our Integrated Care Teams, which also include physicians, advanced practice clinicians, community health workers, patient experience navigators, social workers, Registered Nurses, psychiatrists, and pharmacists. Together this team is responsible for providing and coordinating holistic, patient-centered care for an intimate panel of patients with complex medical, behavioral health, and social needs.

As an Accompany Health Pharmacy Technician, you will have the opportunity to reshape healthcare delivery for underserved patients. You will work with patients and providers to perform outreach to support our pharmacy program, medication adherence program, and value-based care for patients with clinical quality needs. The technician will collaborate with the primary care team to deliver a wide range of clinical services for referred patients, assist in coordination of care with external pharmacies and providers, and help identify barriers to medication access and adherence.


Responsibilities will include:
  • Working directly with patients, pharmacies, and providers to assess medication adherence patterns, identify factors contributing to non-adherence, and support the medication adherence program
  • Coordinating adherence strategies to meet patient needs including:
  • Transitioning patients to 90-day & mail order prescriptions
  • Synching medications to refill at the same time
  • Providing refill reminders for patients and caregivers 
  • Educating patients on the importance of medications
  • Working with patients and pharmacies to ensure medications are picked up and taken as directed
  • Addressing medication affordability through use of generics, formulary medications, low cost pharmacies, and charitable programs
  • Engaging with providers to coordinate prescription refills and renewals
  • Engaging with pharmacies to coordinate prescription fulfillment
  • Proactively identifying members who may be at-risk for medication non-adherence 
  • Serving as a resource to the care team for medication-related inquiries pertaining to formulary tiers, prior authorizations, formulary exceptions, and cost-sharing
  • Scheduling member appointments with our clinical pharmacist for medication adherence issues or medication reconciliation
  • Documenting patient care activities in the electronic medical record (EMR) to track clinical pharmacy interventions and oversight responsibilities
  • Reconnecting patients with the primary care team, if necessary, to help coordinate the completion of their quality health maintenance
  • Working efficiently with multiple providers and providing remote coverage of other markets as assigned
  • Continuously learning and adapting workflows to improve patient care
  • Assisting with other patient care and quality initiatives as needed to promote positive health outcomes and support our health care team

  • Desired skills and experience:
  • Required
  • High school diploma or GED
  • Nationally Certified Pharmacy Technician (CPhT) with at least three years of work experience in this field
  • Strong customer service skills
  • Strong interpersonal skills with the ability to engage with patients in virtual and in-person settings 
  • Ability to build and maintain meaningful relationships 
  • Strong desire to work closely with underserved Medicare and Medicaid populations
  • Experience with navigating Medicare Part D and Medicaid prescription coverage (pharmacy help desk, formulary, prior authorizations, etc.)
  • Experience with pharmacy-related quality programs and medication adherence
  • Experience working on an interdisciplinary care team in a healthcare setting
  • Ability to work during normal business hours (M-F 8AM-5PM EST)

  • Preferred 
  • Prior experience using an Electronic Medical Record (EMR) (Athena knowledge is a plus!)
  • Previous call center or telephonic experience
  • Bilingual in Spanish, Arabic, Portuguese, or Haitian Creole
