PRN LVN- Home Health

We are seeking a dedicated and compassionate PRN LVN to join our home health care team. As an LVN, you will be responsible for providing skilled nursing care to patients in their homes under the direction of a Registered Nurse or Physician. Responsibilities:

• Perform comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the interdisciplinary team.
• Administer medications and treatments as prescribed by the physician.
• Monitor and assess patients' condition, response to treatment, and progress towards goals.
• Provide skilled nursing care, including wound care, catheter care, and tracheostomy care.
• Educate patients and their families on disease management, medication management, and self-care techniques.
• Document all patient care activities accurately and timely in electronic medical records.
• Communicate effectively with the interdisciplinary team, including physicians, therapists, and other healthcare professionals.
• Maintain compliance with all regulatory requirements and standards of practice.

Qualifications:

• Current LVN license in the state of Texas.
• Minimum of one year of nursing experience, preferably in home health or a similar setting.
• BLS (Basic Life Support) certification required.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Reliable transportation and valid driver's license.
• Ability to travel to patients' homes within the designated service area.
