Participates in the development or revision of departmental policies and procedures, and planning/conducting in-service education programs.

Responsibilities
Identifies patients at nutrition risk to prioritize nutrition care for those patients at highest risk.
Utilizes medical record to obtain appropriate/pertinent information for patient care.
Assesses patient nutritional status according to current standards of care.
Calculates nutrient needs, including calories, protein, and fluid, as appropriate to meet individual patient needs.
Calculates tube feeding and parenteral nutrition requirements, as appropriate.
Makes recommendations to physicians re: patient nutritional care.
Develops and implements nutrition care plans and reassess for effectiveness.
Documents appropriately in the medical record.
Educates patients/family members in nutrition-specific self-management skills utilizing appropriate materials.
Routinely consults with medical staff and other members of the healthcare team.
Provides guidance/support to Nutrition Services Department as needed and serves as a liaison with nursing.
Participates in multi-disciplinary team patient care conference meetings.
Functions as a nutrition resource for healthcare professionals, employees, and the patients.
Communicates with Nutrition Services staff in providing accurate nutrition care and high-quality patient service.
Takes part in Performance Improvement activities and participates in quarterly reporting.
Participates in the development or revision of departmental policies and procedures.
Participates in the planning and conducting of in-service education programs.
Participates in and supports on-going performance improvement activities of the organization.
Demonstrates knowledge and skills necessary to provide care based on the physical, psycho/social, educational, and age needs of the patient served.
Performs other duties, as assigned
Complies with all telehealth federal, state and local regulations as well as hospital policies and procedures in accordance with of applicable policies and standards of practice.
Provides telehealth from home (distant location): Maintains an office which safeguards private patient information, medical team/operations conversations and PHI access.
Provides onsite medical nutrition therapy/care as required by state regulation and UnityPoint Health.
Qualifications
Education:
Bachelor of Science in Nutrition/Dietetics, & Master’s Degree with Dietetic Internship or CDR approved equivalent experience
License(s)/Certification(s):
CDR Registration as a RD/RDN
Illinois Licensure as a Dietitian/Nutritionist
Iowa Licensure as Dietitian
