Responsibilities
Be accessible by telephone and prepared to respond to calls during all scheduled shifts
Performs shift management in coordination with other ACT staff according to the established policy and procedures.
Documents patient progress to maintain a permanent record of activity according to established methods and procedure and in a timely manner.
Develop and maintain working relationships with other mental health practitioners, case managers, law enforcement officers, school personnel, hospital personnel, and other interested parties.
Provide crisis assessment and will take necessary time with client to determine course of action.
Complete a thorough safety plan with clients if applicable.
Documents patient progress to maintain a permanent record of activity according to established methods and procedure and in a timely manner.
Other duties as assigned
Functions as a practicing clinician on the team and provides service coordination
Coordinates and monitors activities of daily living.
Provides transportation when needed and purposeful.
Supports patients with behavioral health symptom management.
Advocates for patients’ rights and preferences. Work cooperatively with referral agencies.
Help patient with paperwork and entitlement applications.
Document on contact with patients in EPIC.
Qualifications
Education:
Associates degree in Social Work, Human Services, or another Human Service-related field.
Experience:
Experience with providing service to individuals with a severe and persistent behavioral health needs. Must have lived experience and in a state of recovery with ones own mental health diagnosis.
License(s)/Certification(s):
Valid driver’s license when driving any vehicle for work-related reasons. Class D license. Peer Support Training and competency exam within 6 months of hire. Must 21 years of ago to qualify for Peer Support training. Preferred: Peer Support Certification/Family Peer Support Certification
Knowledge/Skills/Abilities:
Ability to delegate wisely, exercise good judgment, maintain good interpersonal relationships, adapt to change, evaluate critically and think creatively. Individual will demonstrate knowledge and skills necessary to provide appropriate care to adult and geriatric patients with severe and persistent mental illnesses. Individual will demonstrate effective written and verbal skills allowing communication with physicians, co-workers, other clinic/hospital personnel, as well as the public. Position requires good organizational skills.
Other:
Use of usual and customary equipment used to perform essential functions of the position. Position requires remote work. Must have access to cell phone during working hours.
