Position Summary: 

The Aftercare Manager is responsible for the administrative and clinical leaders of the EHN Aftercare program. This position will be involved in the coordination, maintenance and scale-up of an evidence-based aftercare program for clients who have completed an inpatient or Intensive Outpatient Program at EHN Canada.

Job Function and Duties:

Program Coordination and Implementation

  • Collaborate with clients and referents, alongside inpatient staff, to ensure seamless implementation of aftercare programming.
  • Lead the execution, administration, and evaluation of the aftercare program.
  • Work closely with the Clinical Director and Program Managers to develop aftercare content that reflects evidence-based best practices.

Clinical Supervision and Support

  • Provide clinical supervision and support to aftercare clinicians, ensuring the effective delivery of high-quality, evidence-based care.
  • Address clinical issues and client or referent complaints, engaging in service recovery as needed.

Collaboration and Teamwork

  • Partner with the Operations Director to design an aftercare group schedule that meets the needs of both staff and clients.
  • Collaborate with EHN's inpatient and outpatient facilities to enhance continuity of care.
  • Work with the National Director, Operations Director, and inpatient facilities to address workflow and client-related challenges.

Quality Assurance and Policy Development

  • Conduct quality assessments through audits and surveys of clinical documentation and processes.
  • Develop and disseminate aftercare policies, protocols, and process flows.
  • Audit aftercare groups to ensure the provision of quality care.

Program Management and Oversight

  • Oversee the onboarding processes for Intensive Outpatient Programs, ensuring the On-Boarding Team operates with quality and efficiency.
  • Manage the Socialization, Stabilization, and Relapse Prevention Groups, as well as the Monitoring Program (Soberlink and SureHire).
  • Maintain and update the Aftercare Dashboard to reflect current program status.
  • Foster and maintain relationships with aftercare referents and coordinate the distribution of aftercare reports.

Other Responsibilities

  • Support the Operations Director in planning and implementing outcome measurement strategies for the aftercare program.
  • Perform other duties as assigned.

Qualifications:

  • Must have a degree in social work, occupational therapy, psychology, or a related field, with registration in a regulatory body; a master’s degree is preferred.
  • At least 5 years of experience in leadership, including leading clinical teams, supporting professional development, and providing clinical supervision.
