Care Liaison advisor 

Xyla Community Services (part of Acacium Group)

Norfolk & Norwich University

Your contract will be over 3 days to include weekend working.

Fixed term contract ( 6 months )
£25,000 per annum

We are looking for a Care Liaison advisor to join our team, where you will play a vital role in our company's success. In this role, you will closely collaborate with hospital discharge teams and care homes to source the best care packages for our clients.

This position offers a unique opportunity to work towards quick and safe discharge plans, supporting families during challenging times.

  • Collaborate with care homes, care agencies, senior health and social care professionals, patients, and their families.
  • Interact with families in a caring and empathetic manner.
  • Act as the liaison between discharge teams, local authorities, clients, and care providers, ensuring timely and high-quality service delivery within the established timeframe.
  • Ensure prompt and precise discharge plans for patients and their families
