Job description

At Health Care for the Homeless, we believe that housing is health care. We're seeking a dedicated and compassionate Housing Services Case Manager to support individuals transitioning from homelessness into permanent supportive housing. This role is a powerful opportunity to make a tangible impact—by helping clients maintain stable housing, navigate complex systems, and build healthier, more connected lives.

About the Role
You’ll work directly with residents in offsite permanent supportive housing, providing intensive case management and wraparound support services. From home visits and benefit navigation to coordinating care and building community, you’ll be part of an integrated team addressing the intersection of housing, health, and equity.

Key Responsibilities

  • Develop individualized treatment and housing stabilization plans through comprehensive assessments

  • Deliver case management services, including income assistance, employment support, and service referrals

  • Provide in-home and community-based support, including transportation/escorts as needed

  • Collaborate with landlords and property managers to address tenancy concerns and build positive relationships

  • Facilitate events that foster community connection and resident engagement

  • Maintain timely, accurate documentation in the Electronic Health Record (EHR) and Homeless Management Information System (HMIS)

  • Provide representative payee services for designated clients

What You Bring
✔ Bachelor’s degree in human services or 5+ years of related experience
✔ Two years of case management experience required
✔ Conversational Spanish strongly preferred
✔ Valid Maryland driver’s license and access to a personal vehicle
✔ Familiarity with Baltimore City and its community resources
✔ Strong communication, time management, and crisis intervention skills
✔ Commitment to principles of trauma-informed care, harm reduction, and equity
