Pay Per Visit: Must be available for 12 visits per work week

🎯 What you’ll Do in this Role:

Home Health Registered Nurse will Provide compassionate, high-quality care by following personalized care plans in collaboration with patients, families, and the healthcare team. Help patients achieve their goals based on their needs and preferences. As a coverage nurse, you’ll focus on delivering care rather than developing care plans, which are created by the Nurse Case Manager. Primary Responsibilities:

  • Conduct comprehensive evaluations of patients' health status, including physical, psychological, and social factors.
  • Assess patients' home environments to identify potential risks and barriers to care.
  • Follow personalized care plans developed by RN Case Managers.
  • Set achievable goals and objectives based on patients' needs and preferences.
  • Communicate effectively with RN Case Managers, physicians, therapists, social workers, and other healthcare providers to ensure seamless care transitions and continuity of services.
  • Educate patients and their families about conditions, treatment options, medications, and self-care techniques.
  • Provide guidance on managing symptoms, preventing complications, and promoting overall wellness.
  • Regularly assess patients' progress toward care goals and adjust plans as needed.
  • Monitor patients’ vital signs, medication adherence, and overall well-being during home visits.
  • Maintain accurate and up-to-date patient records, including assessments, care plans, progress notes, and communication logs.
  • Document interventions, outcomes, and any changes in patients' conditions in compliance with regulatory standards and organizational policies.
  • Work closely with RN Case Managers and interdisciplinary teams to address complex patient needs and optimize care outcomes.
  • Advocate for patients' rights, preferences, and access to necessary healthcare services.
  • Participate in quality improvement initiatives to enhance home health services and ensure regulatory compliance.
  • dentify areas for improvement and implement evidence-based practices to promote patient safety and care excellence.

 What You’ll Need:

  • Registered Nurse (RN) license to practice in the state of Michigan.
  • Degree in Nursing, bachelor’s degree in nursing (BSN) preferred.
  • Minimum of one (1) year of nursing experience.
  • Previous experience in home health or related field preferred.
  • CPR certification or willingness to obtain.
