Position is remote, 100% tele-health. 

**All candidates residing in Mountain, Central, and Eastern Time Zones are welcome to apply- but must be able to work Monday-Friday 11:30AM-8PM Pacific Time! and 1 Sat shift a month**

Bend Health Coaching Services are designed specifically to extend more helpful resources to more families who need help managing the ups and downs of everyday life. As the research shows, mental health coaching is an effective way to manage emotions, challenge negative thinking, and reduce stress and anxiety.

We’re looking for a Behavioral Care Manager (BCM) to join our IOP team. In this role, you’ll serve as the primary coordinator of care, ensuring that each member and their family has a seamless, supportive, and effective treatment experience from admission through discharge.

Position Summary

As a Behavioral Care Manager supporting the Virtual Intensive Outpatient (IOP) , you will oversee the full lifecycle of care coordination for members in our virtual IOP. You will provide admissions support, including screening and qualifying prospective members for program eligibility, conducting clinical outreach to families, and onboarding new members. Additionally, you will manage case coordination, facilitate communication across multidisciplinary teams, and support members and caregivers throughout their IOP journey.

This position reports to the Virtual IOP Clinical Consultant.

What You'll Do

Admissions & Clinical Outreach

  • Conduct clinical outreach to families to provide education about the IOP services and determine interest in participation.
  • Screen and assess prospective members for eligibility in the IOP, ensuring clinical appropriateness for the intensive level of care.
  • Gather and review clinical documentation to support eligibility determinations.
  • Guide families through the admissions process, addressing questions about programming, insurance, and expectations.
  • Ensure timely scheduling of intake assessments once members are qualified for IOP enrollment.

Biopsychosocial Intakes

  • Conduct comprehensive biopsychosocial assessments for members entering the IOP, gathering relevant clinical, psychosocial, and family information.
  • Orient members and their families to the IOP program, including group schedules, program expectations, technology setup, and available services.
  • Provide education on financial responsibilities, including estimated costs and insurance coverage, to caregivers.

Member Care Coordination

  • Develop individualized care plans in collaboration with the treatment team, ensuring alignment with treatment goals and insurance requirements.
  • Coordinate group therapy schedules, individual therapy, family therapy, and psychiatric services, ensuring smooth scheduling and participation.
  • Serve as the primary point of contact for members and families, offering regular updates and proactive case management.
  • Coordinate external case management needs with schools, Primary Care Providers (PCPs), and community resources.
  • Assist with discharge planning and transition to lower levels of care or additional services, collaborating with internal and external teams.

Transition Management 

  • Notify Core Care teams when members transition into and out of the IOP, ensuring continuity of care across programs.
  • Communicate IOP program requirements and expectations with Core Care team members and caregivers to ensure alignment.

Documentation and Compliance

  • Maintain accurate and timely documentation in the electronic health record (EHR), ensuring adherence to organizational, regulatory, and compliance standards.
  • Complete and upload all required assessments, safety plans, and diagnostic documentation in a timely manner.
  • Monitor member progress and update care plans as needed based on treatment team input.

Program and Team Support

  • Support group facilitators by managing member needs during sessions, including conducting real-time risk assessments if indicated.
  • Provide session coverage (individual, family, or group therapy) when the assigned clinician is unavailable.
  • Facilitate scheduling of all required services, including psychiatric appointments, individual therapy, and family therapy.

Collaboration and Communication

  • Act as a liaison between families, the IOP treatment team, and external providers to ensure coordinated, wraparound care.
  • Collaborate with internal teams to proactively identify and address barriers to care, optimizing the member experience.

  • Technology Proficiency & Telehealth Navigation – Utilizes electronic health records (EHR), telehealth platforms, and digital tools to manage case coordination, documentation, and member communication efficiently.
  • Crisis Management & Risk Assessment – Identifies and responds to safety concerns in real-time, conducting risk assessments and implementing appropriate interventions to support member stabilization.
  • Leadership & Team Collaboration – Works collaboratively with cross-functional teams, provides support to peers, and contributes to a culture of continuous learning and clinical excellence..

Who You Are

  • Master’s degree in Social Work (MSW) Required
  • Experience working with pediatric and adolescent populations, including families.
  • Tech-savvy, with the ability to navigate telehealth platforms and EHR systems.
  • Highly organized with strong attention to detail and the ability to manage multiple priorities in a dynamic, virtual care environment.
  • Warm, empathetic, and engaging communication style that builds trust with members and families.
  • Skilled in case management, care coordination, and collaborative care delivery.
  • Comfortable with evening hours to accommodate member needs.
  • Bilingual abilities are a plus!

Work schedule: Monday-Friday 11:30AM-8PM Pacific Time and 1 Sat per month

The salary range for United States residents is $55K - $75K.
